Specimen received in container labeled as XXXXXXX.
Received single soft tissue mass measuring 4x3x2 cm. outer surface is
lobulated. Cut section is grey white. Representative sections submitted in
cassettes A-F:
A-B: One lymph node
C-D: One lymph node
E-F: lymph node
Microscopic Description:-
Sections show enlarged lymph node with complete effacement of architecture
and replacement by sheets of monomorphic cells. These cells show enfolded
nuclear membrane. Nuclear chromatin is moderately dense. Few mitotic cells
are seen. Extension of cellular infiltrate into perinodal fat is present.
Immunohistochemistry:
Cells show diffuse positivity with CD 20 and CD 10. Few CD 3 positive
reactive T cells seen. Cyclin D1 is negative. Proliferative index is about
35% in focal areas.
Comment:
Histological features are those of non Hodgkin's B cell lymphoma. Suggestive
of a diffuse large B cell lymphoma evolving from follicular lymphoma.
Final Diagnosis:- Right axillary lymph node, biopsy:
- Non Hodgkin's B cell lymphoma suggestive of a diffuse large B cell
lymphoma evolving from follicular lymphoma.

It appears that your dad has got a high-grade lymphoma on the background of a pre-existing low-grade lymphoma. Probably the low grade lymphoma was undetected earlier.
Though this type of lymphoma is potentially curable, with his age and other problems like diabetes, it will be difficult to deliver proper doses of chemotherapy. This could compromise the outcome. Treatment is quite intensive and can cause serious problems like infections.
