Protein/Creatinine ratio is a test that indicates that he has 1.5 gm of protein leaking in to the urine every day (approx.)> In order to tell whether it is abnormal or not for his age group, we require to know few clinical details like
1. Is he a diabetic?
2. Is he having any other major illness like cardiac disease?
3. Is he having or had had any time urinary tract infection?
4. Is he on any drug therapy?
5. Is he a diabetic or and hypertensive?
6. did you ever got the same test done before?
7. What is his creatinine and haemoglobin?
8. what about his kidney sizes (both right and left)?

2. Is he having any other major illness like cardiac disease?Yes he had an aortic valve replacement and bypass one year ago.
3. Is he having or had had any time urinary tract infection? no
4. Is he on any drug therapy? yes. he is taking medicines for hypertension as well as diabetes
5. Is he a diabetic or and hypertensive? yes he is both diabetic as well as hypertensive
6. did you ever got the same test done before? yes it was done 6 months ago.
7. What is his creatinine and haemoglobin?- creatinine -1.8 hemoglobin - 9.8. wbc 0000,
8. what about his kidney sizes (both right and left)?-
additionally his ESR level is at 55, sodium 135, potassium 4.5, chloride 99

Hi
Its very common for the patient with diabetes and hypertension to lose some protein in the urine. His kidney function is less which is seen by his serum creatinine value. He should be going for a nephrologist follow-up. As he is post CABG (bypass) patient, he requires modification of few drugs as per his renal function.
He should be on a statin and aspirin. So that much amount of proteinuria is acceptable. The treatment for such an amount of proteinuria would be ACE inhibitors or ARB. I think , he must be on those drugs which will take care of his renal functions.
See that he gets his kidney (renal) function tests regularly at least once in 4 months so that a major change in the kidney would be detected at the earliest.
