Question: ,chlorthalidone 20 mg, valsartan 80 mg, Dilantin 225mg, omeprazole dr 40 mg, clopidogrel 75 mg, folic acid 1 mg, vitamin D, prenatalmultivitamin, fish oil. Please review and tell me if they are harmful all together.

Dear Ms, I have gone through your question and in understanding your concerns, unfortunately, several drug interactions exist between your current medications that I will list below:

1) Using chlorthalidone together with valsartan can lead to decreased potassium levels in blood so you need to close monitor the level of potassium in your blood regularly

2) Using valsartan with Dilantin can lead to minor decrease of Dilantin effectiveness

3) Using valsartan or chlorthalidone together with omeprazole can lead to hypomagnesemia which is decreased magnesium level in blood. that is why you need to monitor your blood magnesium level regularly.

4) Co-administration of omeprazole and clopidogrel can result in significant reduction in clopidogrel's active metabolite levels and antiplatelet activity. this means that you will not get the full anticlotting effect of clopidogrel

5) Co-administration of clopidogrel or omeprazole with Dilantin may require dose adjustment of Dilantin as both can cause increased levels of Dilantin.

So if you experience any drowsiness, visual problems, behavior changes or poor muscle coordination while using them together then you should decrease the Dilantin dose as soon as possible after discussing with your doctor.
