Do Fever, Cough And Pus Cells Seen In A Urine Report Of An Infant Require Immediate Attention?

Question: My son who is 8 months old has a history of UTI due to Hydronephrosis Grade II VUR with mild kidney scarring. Recently he suffered a Breakthrough UTI.

Since day before yesterday evening he is having cold and cough and since yesterday he is having slight fever of around 99.9 deg F and in the evening today he had 99.4 deg F. We have administered 2 ml of crocin yesterday night and today night. We have given urine sample for Urine Analysis and also for Urine Culture. The analysis report is out and he has Pus cell range of 3-4.

Keeping in mind his medical history, kindly suggest if hospitalisation on urgent basis is required?

Hi!

I have gone through the query and do understand your concern.

I have seen the report and it shows presence of 3-4 pus cells and there is no other abnormality.

Presence of urinary symptoms, Nitrite and RBC with even small number of pus cells may suggest infection. 3-4 pus cells can be found without any infection and can be there even in normal person.

So I don't think that urine report is suggestive of any infection.

Most common cause of fever with cough and cold in this age is some viral infection which improves with symptomatic treatment.

I assume that based on diagnosis of hydronephrotic with VUR baby must be getting some antibiotic prophylaxis.

I suggest you to consult a pediatrician for evaluation and to observe for any abnormal symptoms like:
- Persistence of fever even after medication or for longer duration(more than 72-96 hours),
- Worsening of cough,
- Poor feeding or breathing difficulty which indicates that urgent treatment is needed.
