Dear XXXX,

Fibroid of the uterus is a tumour of the uterine musculature. It is Oestrogen dependant. Hence it grows till the woman has attained her menopause. After menopause, the oestrogen levels starts decreasing and hence the fibroid(s) also starts decreasing in size. Usually, Fibroids exist in multiple seedlings along with the main large fibroid.

Fibroids of the uterus are known to co-exist with the cancer of the lining of the uterus (endometrium) in 30 % of the cases. Hence, before undertaking any surgical treatment of Fibroid, it is a dictum to study the endometrium to rule out endometrial carcinoma or pre-cancerous growth of the endometrium.

Since, after menopause the fibroids regress in their size, there is no chance of bleeding from benign fibroid. If there is any bleeding after menopause and if you are having Fibroid uterus, following two conditions need to be ruled out:

1. Carcinoma of the endometrium: Microscopic study of the endometrium is indicated for this purpose. Hysteroscopically directed endometrial biopsy is the most advocated means for this.

2. Sarcomatous changes in the Fibroid: In fact only after the histological study of the tumour can confirm this diagnosis. However, ultrasonography, Laparoscopy and Hysteroscopy can give some clue about this condition.

Please report to your Gynaecologist to undergo the above mentioned investigations and clinch the diagnosis. Please upload the reports and the images for my review. Prima facae it seems you will have to sacrifice your uterus if you are having postmenopausal bleeding and have a history of Fibroid Uterus.
