Question: I have been having pain in my right side for 2 months now..it is into and under my right rib..I also have microscopic blood in my urine along with mucus with elevated rbc of 3-5 and my white blood count is high..I have been told I have costochondritis..I don't think this is what I have at all..They have done an abdominal/pelvic CT and it didn't show anything..What could this be?

Brief Ans - Pain right under rib? Costochondritis - Under Evaluation

Hi Dear, thanks for your query to HCM Forum.

Read your facts of query and understood your health concerns.Feel Concerned about these health concerns.

I would need further information to pinpoint the exact cause for your urine microscopic showing blood and mucus-

Do you have any colicky pains off and on for last 2 mths?
Do you had any hematuria-blood in urine(I mean do you see the urine blood-tinged /reddish) ?
Any burning micturation in last 2 mths?
Any fever ,loose motions, bloating in right side of stomach or in stomach itself in last 2 mths?
Any problem with Constipation / diarrhea in last 2 mths?
Your question is right- as when CT abdomen/pelvis is normal ? Also
when you have blood in urine micro with mucus?why chondritis? Why not Kideny?

Dear,I would be waiting for more information as asked above, after which I would suggest the right cause of your right rib pain with right hypochondriac pain/or upper abdomen pain.

Welcome for any further query in this regard

Good Day!!

Brief Answer:
Need a few more information.

Detailed Answer:
In this scenario, I would still need more information on
When you had your last menses? Is it normal?
Whether urinalysis was done while the menses was on/of before the menses / or how many days after the menses?
Any problem with low back ache?
How about your appetite-is it reduced lately with constant pain on right side?
Any accompanied nausea / acid reflux with it?
What sort of job schedule and diet you have over a day?
Can you update scanned copies of your urinalysis report and blood test reports?
Await for further information as asked above

Differrent Possibilities for your age - which could cause similar complaints, are as follows-
a-Chronic Rt pyelonephrits-with UTI(Urinary Tract Infection) -which would explain your little RbC and high WBC count
b-Chronic Cholecystitis is another possibility, which may cause complaints and the investigation scenario as described by you.
c-Chronic Hepatitis-need to kept in mind
d-Rt Hepatic flexure Colitis with
e-CostoChondritis-in rt.side ribcage,as told by your doctor.
As the perception of your doctor is based on the physical findings,to which I don't have access,I would suggest you to rely on their advise more and should discuss the above possibilities with your doctors there. and request for more investigations.

Second opinion from Surgeon and Urologist, after physical examination and investigation, would assess the above and other causes and would resolve your health issues.

Suggestions to Manage your health problem-
Being without any information on the above facts in your case,I would suggest you to check up with Second opinion from your Gynec doctor and reassess your right side under rib pain with stomach bloating.

-If you have tested your urinalysis any time during or immediate pre or post menstrual period, there are chances of high rbc increase in such a case,so You should not be much worried.
-Besides this as you don't have any significant urine related complaint, as submitted by you, it could be due to the Pelvic inflammatory disease, which is common in women and needs to be ruled out from your Gynec doctor.

Remedy Suggested on the basis of the available facts in the given situation-
To make you more comfortable I would suggest a Second opinion from Gynec and Urologist, which would resolve your ambiguity about the reasoning of your right under rib pain with bloating in stomach.

Get the following drug trial under your GP doctor-
-if you get relief with it,its Ok.
But if you don't get relief with this drug trial, then go for Second reviews and repeat urinalysis from a different lab,for re-verification.
-A course of Tab Ciplox 500 mgm x 14 days
-Tab Advil 400 mg x 3 Days after food
-Tab Volez D-1 x 2 times per day x 10 days
-Tab Meftal spas-1 x 2 times per day x 10 days.
-With Plenty of fluids

My Impression about your case-
Based on the facts of your query and your lab reports and CT reports,
You seem to suffer from Colitis? with ? PID(pelvic inflammatory disease-causing rbc in urine from low grade bladder- UTI with PID)
-and would advise Stool Examination with Colonoscopy if need be,after the drug trial only.

Reasons of -Why your doctors labelled your problem as Costochondritis-
a-As the pain is located in that region as submitted by you.
b-As the CT abdomen did not detect any other problem with Gall Bladder/ Liver/ or even with your kidney.
c-As you don't have direct urinary complaint with your urination
d-As you neither have any low back complained in your query.
In this scenario-it was a apt conclusion and not to be negated on the grounds you have asked and worried about it.

Solution in the given situation-till you provide Me more information as asked above-
In this scenario,I would suggest you to accept the decision of your doctors and perhaps take the treatment for Costo-chondritis.
Wait and watch till treatment for Costo-chondritis is completed.

If the pain still persists, then you can use the option given by ME as above .
So be patient and go by the way suggested above.
Don't get panicky on this. You can always have another Urinalysis and Second opinion from your Gynec doctor and Urologist,if need be.

Hope this reply would resolve the worries and anxiety you have about the confusion created from the prima facie ambiguity.

Hope this reply would help you to get the right treatment for your health problem, in the best possible way as suggested above by Me.

Keep Me updated if need be.

If you have any further query,I would love to help you out.

If no further query,I would appreciate You,to write excellent review feedback comments about my services, just to help the need patient visitor's like YOU,at My virtual Clinic,at HCM.
