I have noted your sister's clinical details. Prolonged surgery or anesthesia, if uneventful, does not lead to any brain damage or encephalopathy. However, if there is hypotension (drop in blood pressure) or hypoxia (drop in oxygen saturation) for more than 3-5 minutes during surgery, it may lead to what is called hypoxic-ischemic encephalopathy (HIE). Patients with HIE may appear confued or disoriented. So, we need to check with her surgeon/anesthetist, whether or not there was hypotension or hypoxia.
Regarding metabolic encephalopathy, common metabolic derangement to exclude are- elevated creatinine or urea, low sodium, abnormal liver function tests or blood sugars. We need to look at those blood reports.
