Do The Following Test Reports Suggest Kidney Transplant?

Question: Hello doctor,

Good evening !

This is to ask you if Pre Transplant DSA crossmatch says class 1 (negative) , class ii (positive), PRA class 1 (moderate positive), class ii (Strong positive)...CDC (negative)...what does this mean? Can we go ahead with kidney transplant ?

As an Urologist and transplant surgeon,i can understand your concern.

As per the results of the testing for donor,this transplant should not be done.
It's an indication that,a rejection is the likely outcome,due to incompatibility.

DSA is donor specific antibodies. They're subdivided into further classes.
A class II positive report,suggests high rate of kidney getting rejected.

PRA is panel reactive antibodies. They're also further subdivided into classes.
A positive report again shows,incompatibility between donor and recipient.
Therefore it's easy to understand the reasons,why this donor is unsuitable.

CDC is done to know if,any infectious,viral,fungal diseases is present.
This is for organ and tissue safety,so that the organ's protected.

Thanks Doctor for detailed reply.
I just wanted to know if the recipient is on heavy Antibiotics for the last almost a month. Antibiotics like Vancomycin (500mg), Fortom (1gm), Durataz (2.25gm x 3 times a day), Amoxycilin (1.2 gm x 2 times a day) to treat different infection like Urine infection, Throat infection, Blood infection due to Perma catheter. Are there any chances that DSA and PRA may change if the test is repeated 2 -3 weeks after the antibiotics course is done ?

Also does DSA and PRA change with changing donor ?

Durataz Salt is "Piperacillin and Tazobactam"

That means if we change donor....PRA and DSA might change ?

Both the tests are to check if,the donor's kidney will be compatible for transplant.
If they come as negative,it means that compatibility is there.
Compatibility is to know if the organ is suitable and chance of rejection is present.
If negative,it means a suitable organ and rejection chance is less.

So any potential donor must undergo these tests.
