Does Distal Mild Right Motor Axonal Peroneal Neuropathy Indicates I Have Neuropathy ?

Question: Respected Doctor, Hope you are doing great. This is in continuation with my previous consultation with you.
I have had my Nerve conduction studies which says " Distal Mild Right Motor Axonal Peroneal Neuropathy", Does this indicates i have neuropathy??? I have been suffering from paresthesia such as constant tingling sensation in both feets lower side throughout day and night but gets worse in night. Its been now like 2 years i ve had this. Does the test report suggests that i don't have neuropathy but maybe suffering from RLS indeed ?

I have also read your interaction with respected Dr Erion Spaho.

In RLS, patients have a feeling of pain, discomfort, etc in legs, which gets relieved with moving. NCV studies come normal in RLS. The drug of choice in RLS is either ropinirole or pramipexole. So, if you want to clinically test whether you have RLS (either alone or along with peripheral neuropathy), then, you could stop pregabalin, nortriptyline combination and switch to ropinirole 0.25 mg twice daily. If you feel well with ropinirole alone, then you could be suffering from RLS.

As Dr Spaho pointed out, these medicines are needed for long, and they are usually safe. I also prescribe gabapentin and pregabalin to 100s of patients and they don't report sexual dysfunction. So, I agree that nortriptyline could be the drug causing erection/ejaculation issues.

Thanks so much for your reply doctor, i have had tested Pramipexole .125mg once, took only 1 tablet, tingling sensations disappeared after an hour and remained gone for constant 2-3 days but during this period i had severe nausea feeling accompanied by acidity, even pantoprazole DSR did not provided any relief.

Those 3 days were difficult for me to function and even eat anything. That's why my neurologist stopped the medication on my request as i couldn't continue it, so was then switched to gabapentin, worked for my nights continuous 1 year , that too on 100 mg but always gave me internal shaking side effect , during these overall 2 years, i have tried taking anxiety treatment as well using different SSRI/SNRI meds but all aggravated the symptoms of these sensations like total disaster so couldn't complete the treatment.

Now few months back, i was switched to Pregabalin NT combination which have this nortriptyline of 10mg strength, doc. said it will also help in anxiety so far i wouldn't deny the fact that there is marked relief in anxiety symptoms and also no internal shaking side effect but with time i noticed low libido, ejaculation force strength issues. Neurologist said we will continue this combination for some time and will remove it once effect settles in, now since i am not medically certified, i am not in position to argue as i just want relief in symptoms and i myself told him that i do have long standing anxiety/stress since many years.


I have constant tingling sensations during daytime as well in both lower feet but mild that can be felt, as evening starts the intensity increases gradually as time moves forward, i can confirm, i get relief upon movement or even just standing but if i sit on chair, i get restlessness. This is now normal life for me, i feel so sad thinking about it as to why and how i even got it.

Does this sexual dysfunction gets normal after stopping nortriptyline ? Is this peripheral neuropathy will increase or make me disabled?? i am worried about future, i am just 31.

Thank you for getting back and providing more details.

After having tried pramipexole, gabapentin earlier- I agree that pregabalin and nortriptyline seem to be the best for you.

Sexual dysfunction caused by nortriptyline with revert to normal, after stopping it.

As of now, the neuropathy is mild and the chances of it getting worse in future is very negligible.
