I've been diagnosed with depression (F32) and was prescribed Nexito 5 mg (morning and evening). I've heard that it causes weight gain. I also feel off when I have it.

Having said this I like to inform you that Nexito is largely weight neutral medicine and majority of patients doe not gain weight while prescribed Nexito. Only 5-10 % of patients with sedentary life style gain weight as per my own clinical experience.
It can be concluded that with active life style and healthy eating the possibility of weight gain with use of nexito is very low.

Would the medicine, combined with the effects of PCOS, increase the chances of me gaining weight? Because I see a gradual increase even when being active

Although the PCOS does not increase the chances of Nexito induced weight gain but it can itself can contribute toward weight gain. It can be said that nexito induced weight gain, if any, is independent of PCOS.
As you noticed that the weight gain is there despite being active I feel there is need to increase the physical activity and restrict calories. In case that does not help to the significant level I feel changing the medication will be the next best option.
