The subject patient had been advised medicine for three months and date for revisit was scheduled on 03/07/2017. The patient's condition is progressive. Only one problem is slight trouble in walking. For ready ref.copy of the prescription has already been sent to you. please refer to our earlier mail dt. 05/07/2017.from mail ID- YYYY@YYYY

I am glad to note that patient is better.
Following new medicine is needed:
1. Tab NOOTROPIL 800 mg two times daily (9 am and 9 pm).
Change in dose of one medicine:
Tab SYNDOPA 110 mg two times daily (8 am and 4 pm)
Donamem and ECOSPRIN AV can be continued as before.
Colihenz P can be stopped.
Review after one month.
