Subutex consists of Buprenorphin. The drug is used in variety of conditions especially in opioid dependence.
He has developed panic attacks on increasing the dose of drug. Buprenorphin has potential to cause side effects like pin point pupils, giddiness, nausea, itching, dry mouth, postural hypotension.
Sudden increase in dose might result in postural hypotension which may result in symptoms of giddiness and anxiety. Panic attacks could be due to increased dose.
I would advise you to check his pupils in a well lighted room. If they are pin point small and poorly reacting to light then most likely his doses are in higher range. Secondly get his BP checked in lying down position and then immediately in standing position. See the difference in BP. If there is significant decrease in standing position then his problem is due to postural hypotension. This is time to decrease dose and consult his doctor again
