Does Congenital Heart Disease Cause Complications In Becoming Pregnant?
Question: Hi Sir, I am having " Congenital Heart Disease " by birth and till now i am not having any issues due to that and now i am planning for get pregnancy so i would like to know will it be affected my pregnancy and will it be create any problem for me at the time of delivery of before any month.
And Just now as per Gynecologist, we are taking a tablet for Obimet 500. Will it be any harm this tablet?
Reports i will attach here for your verification. So kindly suggest your great opinion. some of the doctors are advised that since it is a Congenital Heart Disease so no problem at the time of delivery too and some of them are suggesting to look into the Multi Specialty hospital. So please confirm me that Can we plan our pregnancy?As per analysis of the attached ECHO report and please provide your valuable suggestion for further action required / Causes / baby planning and if i conceived will be any problem or not?

Dear Madam
You have severe pulmonic valve stenosis with a peak gradient of 113 mmHg.
It is recommended that young adult patients with pulmonic valve stenosis with a catheterization peak-yo-Peak gradient of more than 40 mmHg should undergo ballon valvotomy. All those who are symptomatic and have a XXXXXXX gradient of 30mmHg or more should undergo valvotomy. The data on ECHO gradient is not profound but it is suggested that pre-pregnancy valvotomy should be done if patient is symptomatic or has a peak ECHO gradient of > 50mmHg.
Those who got pregnant with pulmonic valve stenosis tolerate the pregnancy well even if it is severe. In one of the latest studies available in the literature, only one out of 17 patients deteriorated during pregnancy but recovered without intervention or serious complications. Fetal outcome was unaffected. As per literature if patient gets severely symptomatic during pregnancy, balloon valvotomy may be done with protection of foetus, preferably in second trimester.
Since the decision to intervene does not depend only on ECHO report and requires clinical assessment too, you must consult your cardiologist.

As per standard literature, diabetes in pregnancy should be managed with insulin. Metformin should only be used in pregnancy when clearly needed and potential benefits exceed harmful effects.
There is no clear direct effect of metformin on pulmonic valve stenosis as per standard literature, although we avoid metformin in patients in heart failure as these patients may be abnormal renal function. Hence the physician who is prescribing you the drug should be aware of your heart condition and the treating cardiologist should also know that you are taking metformin.
The final decision will be best taken by your gynecologist.

Many Many Thanks for the great response and result and i will be immediately inform my Gynecologist for my Heart Problem too and get a opinion from him. So i hope Metformin is not added in OBIMET 500 . Am i Correct Sir? If dont hesitate and please clarify me clearly that if you feel Merformin is added in OBIMET 500 i will be stop immediately. Please suggest.
