Question: Does this spot look cancerous and need further investigation? I will add a picture.
I pick at it, but it has been bleeding and healing for about a year now. It gets flakey once it heals with a bump under it. Feels like scar tissue.
It is not painful and does not bleed unless I pick at it.

I had a look at the image attached.
Based on the site and appearance it looks like folliculitis. There is no need to worry but I would not recommend to intervene or picking on it. Irritating the lesion can make it recur or stay for long.
You can try applying Adaplene gel over the lesion to get rid of it and prevent recurrence.
