Pyridostigmine is mainly used in the treatment of myasthenia gravis. Myasthenia can mimic and present initially as blephoraspasm so MG workup should proceed and be ruled out.
Pyridostigmine in itself can cause blephoraspasm as a side effect.
As botox is the recommended treatment which can not be used due to the concerns that you mentioned, lid hygiene and prevention of blepharitis should be done. Dryness of the eyes should also be prevented.
Surgical options such as myectomy and superior cervical ganglion block are also available which you can discuss with your physician.
