I have to admit, this is my first query of this nature. The facts around vitamin D do not support this reaction as your doctor so kindly noted. It has been observed that dosages up to 10,000 IU per day of D3 can be taken quite safely.
It is a fact that long term deficiency of vitamin D may cause pain when supplementation is increased and remodeling begins to occur. I will reiterate what your doctor has said, with a normal calcium and magnesium, this reaction should not be occuring at 4000 IU D3.
Now, you have already tried a different composition of repletion and that didn't work. Have you tried injections?
The next step I would suggest is to see an endocrinologist. You realize that Vitamin D is a pro-hormone? It is deeply interconnected with estrogen metabolism, with the functioning of the parathyroid glands, and it sounds like you may need to consult with someone who specializes in hormones.
This sounds like it may involve processes that I have no labs for. I suggest an endocrinologist run some tests on kidney function, parathyroid function, estrogen metabolism and make the determination on how to get your vitamin D regulated and if there are any co-existing conditions impacting on this problem.
