Brief Answer:
Quite normal feeling.

Detailed Answer:
Hello,
It is normal to feel the stab feeling when you have an IV.
The stab feeling is caused by the damage of nerve endings on the skin and vein where the needle is inserted. The ankle of the injection is not important.

Hope I have answered the question.
Let me know if I can assist you further.
Thanks.
