Brief Answer:
Please define your events more precisely

Detailed Answer:
Good morning. I am a neurologist/headache specialist from XXXXXXX Ohio and would like to address your question on the headaches.

It would be more helpful if you could define the headaches you've experienced that you're calling "migraines" since that is a formal diagnosis of a headache type. Do you typically suffer from such headaches and these 3 episodes simply represent an unusual increase for you from baseline since starting this new job which has you concerned? Or are these headaches completely brand new, never before seen or felt, which you or someone else may be referring to as migraines?

Is there nausea, vomiting, extreme sensitivity to light and/or sound, one sided throbbing and intensely painful headaches lasting 4-72 hrs? Have you had at least 5 episodes of the above types of headaches?

If your headaches fulfill the above criteria fully then, you almost certainly have migraine headaches. If you fulfill the criteria partially then, you MAY have migraine headaches.

If the smell of the shop or tires you work with consistently result in the triggering of your symptoms which cascade over a period of 5-60 min. and ultimately lead to the distressing headaches you refer to as migraines then, you certainly may have identified a TRIGGER of your headache. However, if that smell/odor or environment only SOMETIMES seems to be related to the generation of the symptoms or headaches then, it's a little bit of a harder sell to say that you've got a positive trigger. There could be other things at play which bring on the headaches which are yet not identified and the smell may simply be a catalyst which under the right conditions initiates the proper "firing sequence" and once that's been started...it rolls to the headaches.

If you would like to track these headaches using specific parameters in order to define them and then, suggest a treatment or intervention (one of which may have to be the recommendation that you not work at this place) then, let me know and I'll be happy to send you a copy of the HEADACHE LOG that all of our headache patients use to keep track of things in order to identify and correct such episodes. You would simply provide me an email address and I'll forward the log.

I'd appreciate the favor of your providing a STAR RATING and some brief written feedback if your question has been satisfactorily answered. In addition, CLOSING THE QUERY on your end will also be most helpful.

I do have all of the above symtoms and have had Migraines for the past15 years but not this often. I am sensitve to light, i cannot see, i get a headache on 1 side, i throw up everytime. I have worked at a tire place previously and experience the same thing!! So i guess i already andwered my own question!! They do disrupt my work!! I do have medication called Butibiltal, after taking that at onset, it takes approximately 45-1hr before the flashing lights go away, them comes the headache, then the nausea/throwing up!!! It lasts 4 hrs, sometimes all night!!

Brief Answer:
All the more reason you need to fill out a HEADACHE LOG

Detailed Answer:
I see therefore, your experience with this type of headache. As a headache specialist who takes care of "the worst of the worst" at the VA Medical Center I can tell you that we have outstanding success filling out the headache diaries by the patients. This gives us a ton of valuable information which helps characterize, diagnose, and to even predict the types or even specific medications likely to be of use to a patient.

I can't remember the last time I used Butalbital for any of my headache patient. It is not a cure for anything, has dependent properties, can cause severe medication overuse syndrome which tends to give people MORE headaches rather than less. In your case I am prepared to say that it is not doing any good whatever (despite your number of 45 minutes) and should be revised or replaced. You will need something not only to treat the acute episodes of headache but something to prevent them from coming on. The Butalbital is likely causing an entity referred to as MEDICATION OVERUSE HEADACHE.

Please consider my sending you one of our clinic diaries with an explanation by email as to how and fill out the document so we may work together to figure these things out....unless you would simply like to move out of the tire shop to see how that works. But I don't think that will be the perfect solution to your problem.

I hope this answers your question and if so would appreciate a STAR RATING and some written feedback. Also, if there are no further questions your CLOSING THE QUERY on your end would also be most appreciated.

In the future you are always welcome to place questions to me directly by writing to: bit.ly/drdariushsaghafi
