Posted on Fri, 8 Jul 2016
Question: Hello,
Myself- Male, 28 years, In teenage i suffered from OCD (obsessive compulsive disorder), never took SSRI medicines in teenage but took some counselling. Still i suffer from mild OCD.
Problem- My doctor said i suffer from Erectile dysfunction, Premature ejaculation and also Hypothyroidism.
Lab Tests- Total Testosterone 3.86 ng/ml, Total T3- 147.3 ng /dl, Total T4- 7.47 ug/dl, TSH- 16.65.
Medicines Prescribed- 1. Androsar tablets 2. TD Pill 3.Nano Leo 4. L Vate 5. V two powder 6. Thyronorm. also asked me to do sexual exercise like squeeze and stop start technique.
after 15 days no change in premature ejaculation. Hence doctor made two changes to above medicines. removed Androsar tablets, V two powder replaced them Cernos tablets and deplam s10 for anxiety.

My queries- 1. is diagnosis and medicines prescribed correct?

2. I read online that TRT (Testosterone replacement therapy) will cause reduced or zero sperm count. Then after TRT will i become infertile? i am worried because i will marry next year

3. Is my testosterone low for my age? will hypothyroidism treatment address my low testosterone and other sexual problems? Thank you
doctor

Brief Answer:
Please get these tests done

Detailed Answer:
Dear Mr XXXXXXX
Thanks for the query
1- It is inappropriate and unethical to comment on treatment given by other doctor. Every doctor tries to treat to the best of his ability . Doctors can differ in opinion
2- testosterone replacement is safe if some has a deficiency . If given for long periods in people having normal level , it can suppress spermatogenesis. but it is reversible. so don't worry about infertility
3- You should get your testosterone checked between 8-10 AM, give 3 samples and let them mix and find an average value. Please get a SHBG and serum albumin as well because you have thyroid problem already. I shall calculate the active fraction and guide you

Brief Answer:
Pl get the tests done

Detailed Answer:
Hello
Hypothyroidism is related to low testosterone because it causes increase of SHBG (Sex Hormone Binding Globulin) So bio available fraction of testosterone is reduced . Total testosterone may not me affected. PE is related to hyperthyroidism for sure but not to hypothyroidism. I suggest you to get the following blood tests done
- Morning time Total Testosterone
- SHBG level
- Serum Albumin
- Free T3, Free T4 , TSH
What medicines are you taking for OCD . Please assess OCD by downloading YBOCS from net and reading the questions to see if you fit in some of them .
