Does The Following CBC Result Suggest Leukemia?

Question: Hi
I am so very worried I have cancer, in particular leukemia. I am a mess right now. Can you please help and tell me what the chances are that I have leukemia.

I went to the doctor last week because of a feeling of fullness under the lower part of my left ribs.
He ordered an ultrasound, Complete Blood Count, and chest and abdominal xrays.

The report came back. It says I have borderline splenomegaly (measuring 12.9 cm in length).
The blood work came back I also have low platelets - result- 122- range 145-400 x 10E 9/L
The blood work also shows I have low WBC- result - 3.7 - range 4.0 - 11.0 x 10E/9L
My RBC was 4.69 - range 4.20 - 5.70 x 10E12/L
My MPV was 11.1 - range 7.4 - 11.3 fl
I have done blood work in the past that have also said I had low platelets (132 - on XXXXXXX 14, 2014). My platelets have been low since 2012 - they were normal in 2011.
On my blood test in 2014, There is a test I haven't seen on my other blood tests - it says RBC's appear - and the answer is normocytic, normochromic
and another test states. PLT's appear - adequate.
My red cell count in 2014 was 4.98.
What does that mean?

What do you think of my results? Please I am so worried.

I have gone through your question. I can understand your concern.
Your platelet count is slightly low and your wbc count is also slightly low. It is commonly seen in viral infection, malaria, dengue etc. You have only mild spleenomegaly only.

None of the above findings suggest leukemia.

You have gone through peripheral smear examination also. It shows normochromic normocytic rbcs. It means your rbcs are normal in size and shape.
platelet are adequate in numbers.

There are no any blast cells or high wbc count so no possibility of leukemia.
No need to worry. Just be relaxed.

Just repeat your blood count after one month.

thank you so much. what should I look out for?
I have genital herpes. does that matter?
I just noticed that I had a smear done during the blood work last week also - it says - platelets - platelets slightly decreased, rbc's red cells appear normal

Also the report says spleen is bulky and borderline enlarged measuring 12.9 cm in length. Does that normally lead to a diagnosis of leukemia?

I have gone through your question. I can understand your concern.

You should repeat your complete blood count to check total wbc count and platelets count after one month. Genital herpes may the cause of low wbc count and low platelet count. Therefore if herpes is active you should take treatment of that according to your doctor's advice.

Your peripheral smear examination show slightly low platelet count only which we already discussed. No need to worry at all. Just be relaxed. You will be ok within short time.

Just to clarify, I have had lower platelets in the 120-140 range for 3-4 years, but I have also had herpes for 3-4 years.

I have gone through your question. I can understand your concern.
Your spleen is slightly enlarged but there are so many causes like malaria, portal hypertension, congestion, storage disease and many more.

For diagnosis of leukemia, blast cell or high count is must. You don't have any of them. So no need to worry.

Many patients have slightly low platelet count as normal variation. Your count is only slightly low so doesn't concern much. Take treatment of your herpes and repeat complete blood count.

Don't think much about leukemia.

Hi I asked another doctor on here and here was his response. I am very worried again. He was an internal medicine doctor. Do you have any thoughts? Why does he think I might need a bone marrow examination? Would you send me for a bone marrow exam? My platelets have been low for a long time.

See, none of your reports are suggestive of Leukemia. Your platelets and WBC counts are slightly low and if RBC count and Hemoglobin are normal, we will term is as Bicytopenia. Most commonly Bicytopenia associated with spleenomegaly is seen in infections like Viral Infections, Malaria. Most commonly we see this is Ebstein Barr Virus (EBV) infection. I would advice in such scenario, to wait for 2 weeks and get a Complete blood count repeated. If the bicytopenia is persisting, you would require to get a Bone Marrow examination.
However, since your blood picture doesn't suggests any blast cells or atypical cells, Leukemia is very unlikely.
So, stop getting panicky. And get the tests repeated after 2 weeks and write back to me with the reports. Re-open this discussion so that i can get back your history again.

Hi, HPV is a chronic infection, that is very very unlikely to cause Bicytopenia. RBC Count is not important for this evaluation. We need to see Hemoglobin levels. If hemoglobin levels are also low, we term it as Pancytopenia. I would advice you to simply get a repeat blood count done after 2 weeks. If it is still low, then you would need to go for bone marrow examination.

His response is same. He tells you to repeat your complete blood count after 2 weeks as I told you.

He says chance of leukemia are very low with your reports as you don't have any blast cells or atypical cells.what I told is the same.

His response is perfectly right.

Don't worry much and don't panic. Just be relaxed.
Just repeat your complete blood count and give me follow up after 2 weeks.

If your count remains persistently low then you should go for bone marrow examination once to search for the cause.

No need to go for bone marrow examination right now. If needed we will go for it after weeks.
