Question: I am 30 yr old male suffering from varicocoele for last 6 yrs. I recently got tested for s testosterone. the s - testosterone level is 416 ng/dl method is chemiluminescence immunoassay.
I decided to get tested because I was feeling lack of libido and significant decrease in pleasure while ejacualting.. though erection is not a problem the senstivity and pleasure has gone down..
I read that at my age avg testosterone should be atleast mid 600-700.
my questions
1)what should I do about this level of testosterone
2) I will get married by end of year what can I do (including medicines) to prevent further degeneration of testes.

1) Your information is not right. Normal testosterone level is between 300 to 1000 ng/ml in men. So your levels are in very much normal limits.
2) I would like to reassure you that You dont suffer from low testosterone levels and hence related to it like decreased libido.
3) You can safely get married without worrying about testesterone levels and check if you are stressed or having any other issues at workplace or personal life which are affecting you libido.
4) Varicocele will not cause any effect on libido. Please get semen analysis done to see if it is affecting semen parameters like sperm count.

thanks for your quick reply. I had got semen test done year ago and level was 18M. your reply is reassuring however I have been experiencing less intense orgasms lately and pleasure senstivity has also gone down. infact twice in last 15 days I felt nothing when i ejaculated. pls note I was on some antibiotics (sepomax) for cough and uti.
pls advise 1)what can be done to regain same intense orgasms like those of teenage days. I read pleasure at orgasm depends on testosterone levels.
2) I am worried about testicular atrpophy due to varicocoele and its impact on me fathering a child. local doctor recommended fertilovit m plus for genral upkeep and maintenance of testicular health. request your opinion and alternatives if any.

1) Intense and duration of orgasm decreases as age advances. SO you may not feel like you were during your teenage.
2) Pleasure of orgasm or libido does not depend on testesterone levels alone but on your overall health as well as mental stress levels in professional as well as personal life.
3) Varicocele is not going to cause testicular atrophy . Moreover mild varicocele is not a problem.
4) Get married with peace as you have sperm count of 18 million. Then try for pregnancy. If you dont conceive in one year we can also rethink if there is need to operate for varicocele. As of now no need to worry about it.
5) Try to practice yoga and meditation. Reduce stress factors if you have any.
