Done With Liver Screen. What Are The Findings In The Report?

AST - 104 (norm indicated as 10-35)
ALT - 40 (norm 6-29)
LD - 307 (norm100-200)
Also
WBC count stood at 3.6 L (norm 3.8 - 10.8)
The blood test was performed after a fast that began at midnight the night before, preceded by a light dinner.
I am worried, though my doctor has not called me to either reassure me that the results are not a cause for concern, or explain what may be behind these results. I have never before had the test reveal any abnormality, but what I have read about liver screens clearly suggests that the confluence of an elevated AST and ALT does point to liver damage of some sort.
I have no idea what role the low WBC has - or doesn't - in this context.
Do I need further screening? Help, please, I am worried.
Please see the details
Detailed Answer:
Hello! Thanks for putting your query in XXXXXXX I am a Gastroenterologist (DM).
Elevation in AST and ALT suggest that there is some inflammation in liver. And persisitent elevation can lead to fibrosis and then cirrhosis of liver
Some common and treatable cause of elevated AST and ALT are:
1. Alcohol appears to be cause in you as you are taking wine regularly. So I will advise you to avoid it
2. You haven't mentioned about your weight and height. Calculate your BMI from BMI calculator available on Google and see if you are obese or overweight then you need to reduce your weight by doing regular exercise and taking low caloric diet. Otherwise also regular exercise will help.
3.Other common and treatable causes for which you should be evaluated like Hepatitis B and C virus, Serum ceruloplasmin for XXXXXXX disease, ANA and SMA for autoimmune disease
4. Lipid profile if deranged should be treated accordingly
5.Ultrasound abdomen to see for liver echotexture, margins or any nodule in liver, portal vein diameter and spleen size and may be elastography to see for the degree of fibrosis if any
Low WBC count once can be due to fibrosis in liver or it can be erroneous also, so I will advise you to get it repeat if both Ultrasound abdomen and elastography are normal
I hope I have answered your query and this will help you. If you have any further query I will be happy to answer that too. Remain in touch and get-well soon.
