I have gone through your question in detail and I can understand what you are going through.
The usual doses of treating depression and anxiety with mirtazapine is 15mg-30mg. In certain cases when there is no response/less response to 30mg then the doses of 45 mg are used. Your psychiatrist might have increased the dosage as there would have been less than adequate response. If you are feeling worse then getting back to 30mg and then augmenting the mirtazapine treatment with other treatment options like escitalopram or venlafaxine should be a better option.
Also augmenting your mirtazapine with Cognitive behavioural therapy is also a good option for anxiety. This therapy is taken by either a psychologist or a psychiatrist in which the therapist identifies impaired cognitions (Thoughts) and try to correct them. They are 15-20 min weekly session and there should be around 8-10 sessions to help you with your illness.
YOu may discuss these options with your psychiatrist and inform him before you make any changes to your medications.
