1. recently I was diagnosed w diverticulitis
2. I had pain in RIGHT side + fever
3. I was treated with antibiotics + CT scan
4. approximately 4-5 weeks later I had a colonoscopy
5. Dr. said: he saw pus in my colon, and that my colon had 'lots of inflammation' & he gave me more antibiotics bc he said the problem had returned.
5. the official diagnosis was diverticulitis of intestine, unspecified part, w/o bleeding, w/ perforation and/or abscess: 562.11
6. Q#1: how do I know if this is a NEW infection or that the prior infection did not resolve despite the antibiotics?
7. Q#2: after taking this 2nd course of antibiotics, how will I know if the infection has completely cleared?

Diverticulitis is characterised by recurrent bouts of infection and abscess formation. There may be minor infection or collection which might have exacerbated to form larger collection. If you have symptomatically better and subsequent scan shows decreasing volume of collections we could consider that the dissuade has resolved.
You have to get serial scans to confirm to confirm the disease has resolved. Moreover clinical improvement (decreasing trend of fever, pain) is also important. Total white cell count is also a marker of disease trend.

If you have symptomatically better and subsequent scan shows decreasing volume of collections we could consider that the dissuade has resolved.

1. first I had fever and pain on right side of stomach, the CT scan confirmed diverticulitis, and I was given 10 days' antibiotics, after a few days the fever and pain both resolved, also my bowel movements improved, and I started to consistently produce a large quantity during bowel movements daily [this was something new]
2. then 6 weeks later I underwent a colonoscopy, the dr noted pus in my colon, and that my colon had 'lots of inflammation' & he gave me 10 days' antibiotics, the official diagnosis was diverticulitis of intestine, unspecified part, w/o bleeding, w/ perforation and/or abscess: 562.11
3. now two weeks after colonoscopy and after antibiotics, I seem to have no symptoms, I have consistent large bowel movements daily
4. under these facts, circumstances, do you think that the diverticulitis has completely resolved?

Clinically you are completely asymptomatic and disease free.
However diverticulitis is known for repeated bouts of exacerbations and remissions.
For confirmation you have to get a repeat scan to know whether there is any collection left behind it not.

It is the natural course of this condition and the repeated attacks may be because small microscopic level disease persist and it may exacerbate once in a while.
There is no role for antibiotics for preventing exacerbations.
You can make diet changes like high fibre diet and probiotics to prevent repeated bouts.
