Question: Dr. Sharka,

Here are the resutls of the monitor so far. One thing I would like to ask is regarding the fact I have not been doing cardio exercise. Yet, I have had episodes where my hearts are elevated and I cannot breathe properly, would any of these results tell about what is happening during exercise?

Regarding your concern I would explain that some shortness of breath and tachycardia ( including skipped heart beats) are quite normal during straining physical activity.

They are related to the increased plasma levels of adrenaline and noradrenaline as a normal physiological reaction to physical activity.

Anyway, the Holter results will help clarify if it is just tachycardia or cardiac arrhythmia is occurring during those episodes.

I would be glad to review your Holter report as soon as you will be able to upload it!

I reviewed all your uploaded files and would explain that those unpleasant episodes of palpitations, chest fluttering, dizziness are due to repeated episodes of sinus tachycardia but also supraventricular tachyarhythmia.

Frequent episodes of sinus arrhythmia are encountered during the day-night interval; but what is clinically more relevant seems to be the appearance of atrial tachycardia, paroxysmal supraventricular tachycardia. Suspicious runs of atrial flutter are seen as well.

Many of these arrhythmic episodes happen at rest and some of them during sleep and near the midnight. These episodes do not seem to be influenced by physical activity.

Another feature of the ECG recordings is the presence of a specific ST segment elevation, which raises suspicions about J wave syndrome (a risk factor for cardiac arrhythmia). A 12 lead resting ECG is necessary to support/exclude such an alternative.

From the other part, I could hardly see rare isolated extrasystoles without any clinical significance (only 3 or 4 of them).

As a conclusion, I could say that after investigating for possible cardiac structure disorders by means of cardiac echo and if necessary cardiac MRI, a next step would be to look for other possible triggering factors of arrhythmia.

Several lab tests like thyroid hormones level, complete blood count, blood electrolytes level should be checked.

Another alternative to be considered is sick sinus syndrome, but this needs to be proven with an electrophysiological study.

You should discuss with your doctor on the above mentioned issues and a careful differential diagnosis.

Thank you for your insight. I guess now my question is, how serious are these results and how worried should I be? I tend to be a bit of a hypochondriac and of course with the web, I an look up all those terms and be especially concerned. What are some health measures I can take to "treat" these findings aside from talking with my doctor. Thank you.

I don't think that these arrhythmic phenomena are going to threaten your life, but they may disturb you with those intermittent unpleasant feelings.

In general supraventricular arrhythmia is more benign; several extracardiac but also cardiac issues should be sought to find the exact cause of behind arrhythmia. Tha's why I recommended you those cardiac and lab tests.

Besides trying to check those tests, other effective measures to potentially reduce those arrhythmia episodes would be to follow a healthy life-style (avoiding possible stimulants like caffeinated drinks, alcohol, close smoking contacts, etc.), following a physically active daily profile, healthy diet profile.

Brugada syndrome may be excluded by a careful 12 lead resting ECG; you may upload a copy here for a second professional opinion.

At the end, I just advise you to relax and don't worry too much about!

The above situation may be easily managed medically.

So just discuss with your doctor to follow a careful differential diagnosis workup.

The above mentioned issues may serve as helpful clues to arrive to a conclusion.

I reviewed your uploaded attachments and would explain that they show the same arrhythmic phenomena I described you before: sinus arrhythmia, atrial tachycardia, also sinus tachycardia (is normal) and regarding your resting ECG I would conclude it is normal sinus rhythm and the only obvious finding is an abnormal pattern of QRS complex in the inferior leads DII, DIII and aVF.

Coming to this point, I recommend discussing with your doctor on the possibility of performing after the cardiac ultrasound, a cardiac MRI to assure that no subtle cardiac structural abnormalities are present.

Sometimes an arrhythmogenic substrate (focus) is responsible for the above mentioned arrhythmic episodes your have experienced.

So, we have to be careful before getting to the right conclusions.
