Question: Dr. Taka, I'm asking about my husband once again. Lately he has been having some slight headaches and pressure above his left eye (you may recall his history of falling off horse & had a 4mm subdural hematoma July 2018) that we discussed before, as well as transient amnesia one time ( summer 2017) and also off and on optical migraines. But lately, he seemed better since the hematoma absorbed and he didn't require surgery. Except in the past week or two he has had incredible fatigue and some headaches along that left eye. Would these signs (off and on pressure on left eye, and slight headaches & incredible fatigue) still be a concussion so many months later? He had another ct scan in past two weeks, but it was all clear.

Hello again! I am sorry I wasn't able to answer your question a little earlier.

Headache and other symptoms after head trauma can persist for a long time, many months even. That is because often apart from the initial structural damage there are also psychological factors which come into play perpetuating the symptoms. Of course it shouldn't automatically be attributed to the trauma, if he had a period of improvement then some tests should be conducted for other causes. CT scan seems to have been done, some blood tests are necessary as well. Perhaps they were done already but if not I would advise routine blood tests to screen for infections, inflammation and metabolic alterations.

If all the tests turn out normal then I am afraid that it is likely due to the head injury as discussed. In that case if symptoms persist for over 6 months then perhaps preventive medication should be tried, most common ones include antidepressants.
