Question: Dr. XXXXXXX

Who am I? A 47-year women, not pregnant.

Can you help me figure out how to solve the following health problems:
I have been suffering from (1) shortness of breath for the last 6 months, (2) lack of sleep for the last 1 year, but I have always been a light sleeper (Easily awaken by any noise), (3) 30 lb weight loss in the last 6 months.

OBSERVATION: (1) I clear phlegm (or mucus) often that build up in my throat.

I had hives that erupted occasionally on the hands and lips last years that went away, then I noticed shortness got worse. Now, it feels about the same but a little bit better.

I am wondering how does the phlegms (mucus) could be related to my lungs' airways? How do I figure out if there is a lung infection responsible for poor lung functions and secretion of mucus? Could such lung issues lead to lack of sleep, weight loss and lost of appetite?

I took so many tests (such as HIV, Appetites, blood works, X-Rays, PET Scan, lung physical functions, etc) hoping to find an explanation of my sickness, but none of them returned positive. I unfortunately triggered a shingles two weeks after taking a TB test that was also negative. I was told that the shingles eruption is a sign of a compromised immune system, so I am asking myself what infection could be responsible for such illness. The shingles wounds have healed, but I am still in chronic pain for the last 3 months.

I want to know how to proceed to resolve the lung issues that I have asked you perhaps it might resolved all the other issues such as lack of sleep, shortness of breath, weight loss, and stop the phlegm (mucus) from filling up my throat.

I am open to many suggestions that I can explore with a local pulmonologist because I have a feeling that they are not approaching my illness the right way. Too many machines and lab results have been applied on my, but none of them produced results. I believe that there is someone out there, perhaps you who can unlock the secret of my sickness to put me on the path of healing. It hurt me not to know what I am suffering from. Please, help and share information with your colleagues for their opinions and suggestions. Thank you.

Detailed Answer:
Hello and welcome in HCM

I am sorry for your situation and I can understand your concern

It's Dr Nikolla a pulmonologist specialist answering to you.

I read carefully all your medical situation.

If you were my patient there are two causes that should be also excluded as your cause of breathlessness

First.the methacholine test to evaluate your lungs for bronchial asthma and the second less probably the gastric reflux by doing a gastroscopy examination.

Also some blood tests for the allergy such as total IgE, Pneumologia and finally skin prick tests.
I was wondering if you have done a heart control
It should be excluded too.

So please discuss with your doctor for the above and when you have the results you can attach them to me and I will help you again.

Hope to have been helpful

Regards

Is there a less invasive procedure for bronchial asthma than methacholine test and bronchoscopy?

IS THERE ANY mucus test to diagnose bronchial asthma?

Detailed Answer:
Hi again

You don't need bronchoscopy for diagnosis of asthma.Metacholine test is not invasive. It is a Spirometry test.

You can do the examination of secretions for eosinophils which helps in the diagnose of allergic asthma.
