Question: Dr —
1. I recently underwent a peripheral venous Doppler of bilateral lower extremity’s venous mapping
2. My results are as follows:
A) Varicose veins with pain
B) Varicose vein’s with other complications
C) Chronic venous hypertension

3. The impression was as follows
A) Negative for deep venous thrombosis
B) Positive for Venus valvular insufficiency in bilateral great saphenous vein’s
C) Positive for Venus valvular insufficiency in bilateral small saphenous vein’s
D) There is evidence of left calf in competent perforator

4. the doctor wants to perform the following procedures --
A] RGSV, LGSV, RSSV, LSSV, Ablation
B] RGSV, LGSV, RSSV, LSSV, Sclero

5]. My questions are as follows --
A] since I don't seem to have any pain or other symptoms - are these procedures necessary?
b] what would happen [in the long term] if I decline these procedures ?
c] what would happen if I delay these procedures for a year or so until after the COVID-19 pandemic?
d] what would happen if I used non invasive means to treat these problems -- such as compression socks, losing weight, exercise?

Anyway, there is no emergency regarding the procedure.

As there is no evidence of deep vein thrombosis, the situation may wait even a year, considering the actual situation of corona virus.

Anyway, you should know that you are at an increased risk for deep vein thrombosis and pulmonary embolism during this period of time.

For this reason, I would recommend avoiding prolonged sitting or laying down position.

Another way to prevent deep vein thrombosis or pulmonary embolism would be taking anticoagulation therapy during this period of time. In this regard, I would recommend taking a low dose (half dose) of Rivaroxaban (10mg daily) or Apixaban 5mg once a day.

Wearing compressive socks when prolonged standing up position will help improve the pain.

1. in your response you said to avoid -- prolonged sitting or laying down position.

So do you recommend elevating my legs if i lay down [while watching TV] for example?

2. also do you recommend using compression socks while prolonged standing/sitting at the office for example?

3. do you think that -- if i increased exercise & lost weight -- could this problem subside or get resolved without medical intervention with Ablation & Sclero?

4. Finally, are the procedures listed below invasive requiring hospital stay?

Here are my answers to your question:

1- Lifting your legs up would help improve the blood circulation when lying down.

2- The compression socks should be worn during prolonged standing up or when sitting.

3- I don't think that your situation will improve without the need of surgery. The above measures just help reduce the pain and swelling. The walking and exercises would reduce the risk of pulmonary embolism or deep vein thrombosis.

4- All the recommended procedures can be performed within 4-6 hours without the need of hospitalization. It is quite a safe procedure with little pain and you will be able to walk right after the procedure, wearing compression socks.
