Question: Fever between 101.3 and 104 x3. Muscle aches. Headache. Ear pain. Nasty sinus infection I'm guessing. Do I need an antibiotic. How will do determine whether it's viral or bacterial?

I read your query and here is my advice.

Most probably your symptoms of congestion, headache, ear pain, stuffed nose are signs of sinusitis.
Most of sinusitis are viral.
Usually it is not treated with antibiotics. If the infection lasts for more than 5 days and the symptoms doesn't get better but worsen most probably the infection is bacterial.
In this case we use antibiotics.
In order to determine for if it is bacterial or viral a secretions culture needs to be done.
We can use antibiotics empirically in your case.
Nose irrigation with saline water is advisable.
Also decongestants such pseudoephedrine and antihistamines such citerizine are advisable.
