For Dr. Spaho - Doctor, Just A Super Quick Clarification

Question: For Dr. Spaho - Doctor, just a super quick clarification question. When this same numbness happened a few years ago (from trauma), I believe the neurologist said to increase the B vitamins and duration to get some feeling back in area usually ranges between 4 - 6 weeks. Is this correct?

Brief Answer:
Vitamins B group supplements may be of benefit in your case.
Detailed Answer:
Hello again and thanks for asking me directly.
Despite the underlying causes of the small peripheral nerve damage (be it trauma, compression, metabolic, etc.), vitamin B group supplements help in regeneration and, this way, in symptoms improvement.
So, the use of relatively high doses of such supplements for one or two months is justifiable and could help in improvement.
In conclusion, vitamin B complex may be used together with other drugs in your case.
Hope I helped you.
Wishing you good health.

For Dr. Spaho. Hello, doctor and thank u so much for all your help. Your knowledge and answers truly show excellence coupled with knowledge. Need your advice. Prior to going for botox injection, neurologist (as well as reading online) that seeing a special chiro (upper cervical) can sometimes help atypical odontalgia as well as the TN2 I have on the other side from years ago laser gum surgery. I've had 2 treatments with a special chiro (he precisely measured and found I am 8mm off on right side) and is slowly adjusting the atlas bone (C1) I believe. However, the TN2 that was dormant prior to his treatment, has now ignited again ( burning, pulling throbbing, ear issue, etc) along with cluster headaches. According to him, this is common and told to "hang in there". Your opinion, however, is the only one I trust. My question is should I continue on this path to correct the mis-alignment or cease? I am totally at a loss as to what to do now with both neuralgias going in tandem. Thanks so much, doctor for your help. Oh, just a footnote. This chiro uses a very precise machine (no twisting with hands) to lightly adjust that atlas bone. Very specialized but still unsure if this will help either issue.

Brief Answer:
I think chiropractic manipulations should be stopped for now.
Detailed Answer:
Welcome back.
I have read your new question.
If there are new symptoms after these chiropractic manipulations, even if there was found misalignment, I think that you should stop the manipulations.
It is difficult to say if chiropractic manipulations may improve odontalgia since these manipulations aim to improve neck related symptoms.
So, I think that botox injections should be tried for your symptoms.
Hope this helps.
Feel free to ask if you have further questions.

For Dr. Spaho - Thank u for your answer. Customer service said I have only 2 days for follow up questions and an important matter has come up just recently where I have almost totally lost my sense of taste. Almost certainly neurological related. Neurologist suggested increase in Alpha-Lip. However, I have been taking that along with magnesium, B Vit, taurine as supplements for some time now as well as Neurontin. My question is this - can u suggest anything I can do or take to restore sense of taste and also, can taking too much of a supplement/med suppress the sense of taste? Any suggestions u may have would be very appreciated.

For Dr. Spaho - Doctor, just a quick footnote to previous question re: loss of taste. Concurrent to losing sense of taste, noticing an odd metallic taste and wondering if this has any significance. Also, I will be cancelling all future chiro appts but one quick question in your experience with AO. Having it for 40 yrs, it's always been localized (between 2 teeth). However, subsequent to having that area of gum cut approx. one month ago, first exp. numbness and now, that ache radiates into neck and shoulder blade. Before cancelling chiro, just wondering if you've ever seen these symptoms radiate with AO? Thanks so much.

Hello again.
Disorders of taste and smell are caused in most of the cases by viral infections, sinusitis, etc.
Only rare cases are linked to neurological conditions such as Parkinson's, etc.
So, besides the supplements that you are taking, an evaluation for an infection is necessary ( and subsequent treatment).
Regarding the neck and shoulder is not related to AO, but it may aggravate any previous degenerative changes of the spine due to inappropriate posture for example.
These symptoms indicate a pinched nerve in the neck, and, in some cases, chiropractic manipulations may help with such symptoms, but not always.
Hope I have been of help.
Best regards.

Hi, doctor. Just a quick follow up question since I have a long wait yet to get in to see neuro surgeon (just for consultation on getting that injection). In the meantime, ever since the gum was cut in target area about a month ago, numbness has gone but replaced with the atypical ache off the chart in intensity at a 10. The meds I take really don't help. Just wondering if u could suggest any topical (s) I could put on the area to possibly calm down that nerve (any thing at all u could suggest) and also, being in extra trauma, I know it's impossible to say but on balance, how long a duration for it to calm down to possibly a bearable degree? Thank you, doctor.

Brief Answer:
Topical lidocaine may be of help.
Detailed Answer:
Welcome back.
A temporary solution for your symptoms may be application of topical drugs such as lidocaine spray for example.
it may be used several times and with little or not at all adverse effects.
It may cause numbness but will settle down the pain.
It is difficult to predict the time for diminished symptoms, anyway, it may take some weeks.
Hope this helps.
Feel free to ask if you have further questions.

Hi, doctor and thank you again. One important follow up question since the neurologist here did diagnose the loss of taste as being Burning Mouth Syndrome which is as u know, a neurological issue. By ruling out viral, sinus etc he determined that since loss of taste is accompanied by a metallic taste and burning tongue, it is that condition. What I forgot to ask him is two questions maybe u can help me with. Is this a condition that is temporary in nature and also, can anything be done to treat this?

Brief Answer:
Your questions answered below.
Detailed Answer:
Welcome back.
I have read your follow up question.
Burning mouth syndrome is a chronic pain syndrome, complete remission of symptoms is seen only in a minority of patients.
Most of the cases take years to regres.
Since the cause of burning mouth syndrome is not well understood ( some cases are linked with deficiencies of vitamins and micro elements, a number of cases are linked with psychological issues, the remaining there is no underlying cause), treatment consists mainly in the use of supplements, resolving psychological problems, and symptomatic treatment with topical lidocaine, capsaicin, etc.
Hope you found the answer helpful.
Regards.

I'm going to have the pharmacy make me a lidocaine/capsaicin compound.
For optimum use, should I request this in a spray or gel cream? Should this be placed directly on the tongue? If done with diligence, I'm sure it will dull the pain but might it restore taste as well? Seems like a double edge sword since when mouth is numb, taste dulls even more. Thanks so much.

Brief Answer:
In my opinion, gel is better.
Detailed Answer:
Welcome back.
In my opinion, gel is more appropriate to be used in your case.
It should be applied on the pain areas including the tongue.
It may cause some numbness, but is acceptable if it will control sufficiently the pain.
Hope I helped you.
Take care.

Hi, doctor. Customer service contacted me for add'l funds which was gladly given due to the caliber and accuracy of your answers. Case in point - the gel you recommended for burning tongue arrived earlier from compound lab and after one application, burning has subsided substantially. My primary issue now is loss of taste. Due to my history and having spoken to neurologist, this loss of taste is almost certainly neurological. This one is very depressing since I am already very thin and food has even less appeal. I know u said this sense will be gone forever, but I have to hope that possibly it will return. Just wanted to ask if u could suggest anything ( a med, home remedy, anything) I could try to possibly jump start the sensory nerves that control taste.

Hello again and thanks for following up with me.
First, I'd like to correct something: the taste returns in the majority of patients after some years.
So, there is no reason to lose hope.
Since the gel is working for the burning sensation, you should continue supplements of vitamins B, alpha-lipoic acid, and you should be evaluated by blood tests for any deficiencies of iron, zinc, etc.
Clonazepam also may be used for further improvement.
Try to avoid smoking, excessive drinking, maintain correct oral hygiene and the taste will most likely reappear.
Hope this helps.
Take care.

Hi, doctor. I just wanted to thank u for your suggestion on clonazepam. I asked the doctor for the lowest dose of the oral disintegrating wafer and it not only helped mitigate the burning on the tongue but has brought back partial taste. Now if only I could somehow get a handle on over 30 years living with atypical odontalgia non-stop, I would get my life back. Quick question since u are very unique inasmuch as knowing the exact condition of how this nerve fires constantly, causing that continuous dull, throbbing ache between two teeth. (Well, implants in my case). Before I continue grabbing at expensive and time consuming straws, just want to ask if you have ever heard of any kind of chiro treatments (upper cervical where Atlas bone is adjusted) helping with AO in any way? Chiro says he may be able to take pressure off of trigeminal nerve but I'm skeptical since this ache feels imbedded and not even sure it involves trigeminal. Thank you, doctor, and you're worth your weight in gold.

Brief Answer:
Chiropractic manipulations seem not necessary for now.
Detailed Answer:
Hello again and thanks for the kind words.
I have read your new question.
In your case, the problem is not the trigeminal nerve itself, small peripheral branches may be irritated.
You tried chiropractic adjustment and, as I remember, you had new symptoms after it.
Furthermore, chiropractic manipulations are not included in the possible treatment modalities for AO.
These manipulations may help in some cases with neck, shoulder, pain.
So, for now, I don't see a good reason to continue chiropractic manipulations.
Hope this helps.
Best regards.

Once again, thank you and in total agreement regarding AO being a peripheral nerve scenario. Just to summarize, with that being the case, do u still feel the botox injection may be of help for my particular situation, being not directly invoked into trigeminal? And if so, equating risk with gain, are there any adverse side effects or on balance, a relatively safe procedure?
