For How Long Can NSAIDs Be Taken For Treatment Of Psoriatic Arthritis?

Question: Hi I m 31 yrs male suffering from psorosis arthritis from 2003 but...had been in remission for last 9 yrs till jan2013 and got joint pain again for next 10 months and then again got remission for next 11 months. ..now from XXXXXXX 2015 till today having joint pain mainly involve a single hip joint and single wrist and single shoulder

Take etoricoxib 90 mg in a gap of 3 to 4 days when feel pain increasing. ..before had taken sazo 1000 mg for 6 months in 2003 and for 4 months in 2013....

Been to rheumatologists and all had prescribed same medicines I. E sazo 500 mg twice and etoricoxib 90 mg

My question is how long can I take nsaid as they have side effects and how should I proceed further with this and my chances to achieve remission again

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that your treatment is on right track although it will be preferable if you start with methotrexate as it seems more effective than sulphasalazine since your peripheral joints are also involved .

I need to see all your records specially inflammatory markers like ESR , CRP , HLA B 27 etc. Also whether you had sacroillitis or not also matters in your course of treatment . Also biological drugs like Infliximab or etanercept can be taken into consideration.

Generally NSAIDS are safe options if taken in limits , although studies say that it can be used for 2 years at a stretch but it varies form individual to individual . So regular kidney and liver function testing is advised . You can discuss this with your treating rheumatologist .

crp greater then 10...
hla b27 was positive
anti ccp and rf factor ... negative
how can sacroillitis can be ruled out.. and don't want to move on methotrexate due to serious side effects and can I stop medicines again if achieved remission

Thank you for reverting back.

Well your disease seems to be active , since you are HLA B 27 positive the disease will be little bad prognostically . Methotrexate is a very good drug for your disease and we use it quite often and its very safe , only you have to be vigilant for the side effects . Else you option will be biological drugs .

In my opinion you should go for methotrexate ,plus the treatment is long term in these diseases and some amount of medicines will always go on so you be prepared for the same .

Well it just depends on individuals body , there are no comparative studies to suggest the same . In my opinion methotrexate should be tried first , if it fails then only biological should be used .

Sacroillitis can be the cause of your hip pain , it can be seen in MRI pelvis . Your doctor must have seen this already.
