There can be many reasons for going to toilet frequently. These can be ranging from innocuous to serious. You need to tell more about it before we can conclude what the possible reasons are. I would need the following information:
1. How long has this been?
2. How many times a day? - Is it watery or solid?
3. Is there any relation to food? - Does it aggravates with any particular diet?
4. Do you also pass stools at night? Does it wake you from sleep?
5. Is it associated with pain in tummy? Does going to toilet relieves the pain?
6. Is it associated with abnormal sounds in tummy?
7. Are these associated with blood?
8. Is there any weight loss or fever?
9. Are you taking any drugs?

A. Wakeup - 6:30 am, After Morning Tea - 7:30 am, Between 10am-12 noon, Between 3pm-5pm, around 7:00 pm at night it depends, Total 6 times, Motion is solid - sticky.

A. Yes, might be non veg.

A. NO

A. Sometimes

A. No, But sometimes if when feel uneasy I take Pudin XXXXXXX or PanD.

Based on the information provided, it seems you are suffering from some kind of Functional Bowel Disease, probably IBS (Irritable Bowel Syndrome). This is a life style associated medical condition.
I am not sure how much milk you take. You should stop taking milk, as well as juices. Sometimes lactose or fructose intolerance can lead on to similar kind of symptoms. Though you can take curd and butter milk as in these products lactose is already fermented. Be regular with your diet and follow de-stressing measures - regular sleep, physical exercise, transcendental meditation.
If you already following strict diet and life style as mentioned above, then I would suggest you to see your local gastroenterologist who may do a battery of tests like stool examination, complete haemogram, thyroid function tests, tests for Celiac disease, etc to rule out common organic causes of your symptoms. Colonoscopy can give added information about your bowel condition.
An empirical course of antibiotics acting on gut may also benefit sometimes.

In office I have a small cup of cow milk, but at home I don't even then I feel like going to toilet, sometimes when I feel going to toilet when the pressure is there, but the toilet does not come when I am on the pot. What may the reason?
This appears to be due to pelvis that regulate defecatory movements may not be functioning in a co-ordinated manner). This is also a part of functional bowel disease.
De-stressing exercises, yoga etc. should help you with this.
