Good Day, I Am Writing You From Bosnia And Herzegovina,

I am writing you from Bosnia and Herzegovina, we have a son 19 months old with undiagnosed issues, we suppose in a digestive system. After all physical tests: 24 hour ph-testing showed 205 reflux episodes at age of 10 months which medicine proclaimed it is not reflux, but in that period he had clear symptoms which showed he has some problem; then ultrasound performed with milk drinking while child were sleeping showed that he has reflux, it has been seen 5 reflux episodes in 2-3 minutes, then barium swallow (child were getting up during this test because I couldn't hold him strong enough) showed he doesn’t have reflux, endoscopy which has results: cardia gapes, cardia and pylorus are on same level, MRI- clear, so medicine didn't find problem with our child and they discharged us home.
What can I say about his problems/behaviour. He is arching his back while he sleeps and wiggling trying to find corresponding position, then he wakes up, and burps, I can see he swallows when he stands up, he don’t gain weight well, he has now 10.5 kgs, 19 months old, there were period he gained 300 grams for 5 months (in period he was 5 months old, lasting till he was 10 months old), this summer he gained nothing for 2 months. Many exams for reflux were done, some showed no reflux, some showed there is reflux, but anti-reflux medicine didn't help at all. Since he was born he acts like this, he couldn’t sleep, he cried all the time, he couldn’t be laid down till he was 1 year old, he couldn’t sit, when he was sitting he also started to cry, to wave with his hands to lift him, when I would lift him up, he would want to lay down again and he would start wiggling on the bed nervously what lasts also today; he wiggles a lot while he sleeps, and he sleeps on his knees and moves up-down searching corresponding position.
He had plenty of symptoms including coughing, burping in a huge amount, dark, smelly stools, arching and so on... Symptoms he has now also is very loud stomach after eating liquids, when I move him, it can be heard that this liquid is overflowing in stomach with very strange sound. I noticed that 1 year ago.
I have many video to show how he behaves. During the day he looks now almost normal, he has some accelerated actions, which were more expressed when he was crawling or even before when he couldn’t do many moves by himself. I think then he has pain when he is accelerated, what could be more seen over the night.
We are looking for help for our child... We don't find any solution more, but we are convinced that he suffers from unknown cause of pain. Maybe he has lack of some cells, enzymes or something, I don’t know anything anymore, I only know that he has a problem for sure.
Is there any way that You are able to help us?
Thank You in advance.

It seems complex
Detailed Answer:
Hi Marcrnog,
I am sorry to hear about the troubles your child is experiencing. In particular, I notice the symptoms of apparent abdominal pain, previous foul smelling stools and inability to gain weight. Also, what you describe sounds like more general or multiorganic developmental issues, e.g. with arching and inability to sit at age 1.
Has your son been examined for enzyme deficiencis at birth, e.g. defects in amino acid metabolism? In some contries this is performed as a blood sample from the newborns heal 48-72 hours after birth. Any reports or notes from a neonatal or pediatric diagnostic center? Blood tests?
En terms of the gastroscopy that your child had performed, did they examine him for a pyloric stenosis? Any reports from the examination? Typically, they would have taken duodenal biopsies for celiac disease and luminal enzyme deficiencies?
Furthermore, I find it relevant to do an ileocolonoscopy with biopsies with your son in general anesthesia.
Looking forward to your reply.

how can I send you some files with examination results? these are in English, but i can try to translate it. I think there is no enzyme deficiencis took ever.

they did also ileocolonoscopy I think-test with contrast in rectum done by x-rays- clear. They did rectum biopsy, they found ganglian cells-everything ok, except this part: The lamina propria has a chronic inflammatory infiltrate and mild hyperemia. there is also one focal lymphocyte buildup. 4 eosinophils on 1/VVP are found in one focus.
can you precise what is duodenal biopsies for celiac disease and luminal enzyme deficiencies? they did endoscopy, but I don't know why they didn't take tissue sample from the stomach and esophagus?

Blood results in English are fine
Detailed Answer:
Hi Marijana,
You can just upload the blood results, English is fine.
The diseases I mention are usually diagnosed by a heel prick shortly after birth. It tests for congenital hypothyroidism, cystic fibrosis, metabolic disease (abbreviations PKU, MCADD, MSUD, IVA, GA1, HCU), among other.
When malabsorption is suspected, amylase is good to know, but also elastase in the stool. Did the biopsy report on any specific condition they suspected based by the histological examination?
During the gastroscopy, they should - in my opinion - have taken biopsies from the duodenum, as this can help diagnose celiac disease, Whipples disease, saccharidase or peptidase deficiencies, to mention a few.
Looking forward to hearing from you again.

i sent you also x-ray photo of the stomach. I have important question, is the SHAPE and POSITION of his stomach fine? his stomach "lays" a little bit comparing to normal shape of the semi-moon. only results they wrote after gastroscopy is: cardia gapes, stomach shape U, pylorus and cardia on the same level. But they think it is not a huge problem, they gave him antirelux therapy and some medicine to speed up digesting of the food, but without any improvement. I remind you that after he drinks milk from the bottle, it can be heard very loudly milk overflowing around the left ribbs, something is happening... position, and shape of the stomach maybe? Or inside some cells are missing.

Stomach looks normal
Detailed Answer:
Hi again Marijana,
I've reviewed the reports.
The position of the stomach looks fine, there is nothing to indicate any problems here. If antireflux medicine did not help, a "gaping" cardia is unlikely a problem either.
My primary advice is to seek a second opinion in your country, i.e. at another pediatric department. If this fails to get you closer to a diagnosis, you should ask your local political representatives for the possibilities of further examinations abroad.
How does this sound to you?
Let me know if I can assist you further...

sorry, I have just read your last reply. ok, we will try to see some doctors more... we were in 2 hospitals, no any diagnosis...
probably we will see some doctors more, but there is no possibility that they will do some exams more when they see results of the test we already did...
he is much more better comparing 2 months before. i called alternative medicine to help, and he is better from that period 200%, but he is not healthy still... he still has symptoms i described.
I have one more question:
can you tell me please is it normal after every liquid meal (lasts for 1 year) to have loud movement below the ribbs? that can be felt when touching the ribbs.. this is milk overflowing, hits the stomach "wall". i have video which showing it, I can send you if needed. what this means? it lasts at least 1 year

A video would help
Detailed Answer:
Hi again Marijana,
I am glad to hear that he is better; that definitely points to a less severe cause of his symptoms. It is pretty normal that bowel sounds can be heard after a meal, but if it of any concern to you, I would gladly look at the video.
Best wishes.

He looks like a healthy boy
Detailed Answer:
Hi again Marijana and thank you for the videos, he is certainly a handsome and healthy looking boy.
Whipples disease IS caused by a bacteria, but it is diagnosed on biopsies from the duodenum with certain staining techniques (PAS) and not from stool.
Stool elastase is a marker of malabsorption and pancreatic function; not bacteria.
To me he seems like a healthy boy, and, as long as he reached his milestones in due time, it may just be a matter of time. Have you tried switching between different food items or beverages? The gurgling sound from his stomach seems very normal to me, and is something we can all experience if moving quickly after drinking lots of fluid.
Looking forward to hearing from you again.

Colonoscopy and then MRI of his small bowels should be next
Detailed Answer:
Hi again,
I did watch the videos, and he certainly appears to have some discomfort. As I mentioned earlier, considering his symptoms right now, he should have an ileocolonoscopy in general anesthesia and if this does not get you closer to a diagnosis, an MRI of his small bowels could be next.
Best wishes.
