Question: Good day.
My last ultrasound showed the following :
The uterus is heterogeneous in appearance with multiple intramural and subserosal fibroids. The two larger
intramural lesions are located within the anterior bodymeasuring 4.3cm and 3.1cm in maximum short axis. Two
large subserosal fibroids are located anteriorly as well as posteriorly within the lower body measuring 3.5cm and
5.2cm respectively.
The endometrium is not positively identified as it is distorted by these fibroids, however no evidence of endometrial
thickening is seen.
Since Sunday I have been spotting. I am 58 and menopausal. Could this a sign of cancer ?

Brief Answer:
There's no evidence of cancer in this USG report
Detailed Answer:
Hi
Thanks for your query.
There's no thickening of endometrium in this report so chance of endometrial cancer is very little. Fibroids are benign too. Because of distortion of endometrium, there may be spotting. But spotting in postmenopausal woman needs further evaluation.
To find out the exact cause and rule out cancer, I do recommend a hysteroscopy exam.
Hope this helps.
Regards

Dear Dr.
Thank you for tour reply. I did see my obgyn and we have scheduled a hysterectomy for next Friday. I have always had a history of endometrial hyperplasia and after an ectopic pregnancy I have only one polycystic ovary and one tube.
So I have read in my case I am predisposed to cancer of the endometrium. I am just praying and hoping it has not started. In 2014 my endometrium was 7mm which is over the average.
I am very worried.

I should tell you that I did a thyroidectomy in dec2019 so I am on the daily hormone. But I ran out and was not taking any for about 7 days.

Brief Answer:
Yes endometrial hyperplasia may predispose to cancer
Detailed Answer:
But current USG doesn't raise suspicion for cancer. A hysteroscopy will be useful to rule it out. Thyroid is probably unrelated to this.

Okay. Thanks.
I will update you after the hysterectomy. I am not spotting today but I wonder what actually does cause the spotting ?
Is it that the bleeding was caused by cells that became abnormal. Now that those have fully now dead and more cells are now been mutated and in another month or so more will become abnormal and bleeding will again be seen

Brief Answer:
Spotting depends on many factors
Detailed Answer:
sometimes cells are shed then bleeding happens. Then some regeneration is there, so bleeding stops. It's not really related to genetic mutation.

Brief Answer:
Shedding can happen
Detailed Answer:
This shedding is not due to menstrual cycles but due to distorted anatomy.

Is there ever a case where spotting is a latex sign of endometrial cancer

I think the cause is either the hyperplasia has caused precancerous abnormal cells which WILL become cancerous or they are already cancerous. I think the usg did not pickup the thickness properly. If I find that to be so I shall be very unhappy bv with the lab

Brief Answer:
Friday is not late
Detailed Answer:
spotting can occur in benign cases too. Many times biopsy for spotting reveals hyperplasia, atrophic endometrium, polyps, etc. So it's not cancer always. However, there's no specific sign which can differentiate benign from malignant cause for spotting in a postmenopausal lady (apart from endometrial thickness in USG or MRI). That's why we always evaluate with hysteroscopy.
Moreover, you are already scheduled for hysteroscopy and biopsy. You have mentioned hysterectomy which is removal of uterus but I think your doc will see inside first and take a biopsy (hysteroscopy). We usually do hysteroscopy before deciding on uterus removal.
Friday is just around the corner and you should be fine with that date.

Brief Answer:
Please keep in mind that is is more likely to be benign
Detailed Answer:
Don't think about malignancy all the time, Probability of that is anyway lower.
I am located in XXXXXXX
