Question: Good morning, I am 66 years old and have a sexual question. My wife and I are still sexually active about twice a week and I do not have and problems and never have had any problems with erections. I do have a problem with numbness in my feet and now in my penis but I am taking Topiramate 25mg X 3 day and that was helping for a while. The proble is this, I cannot feel when an ejaculation is going to occur. I am not sure it does. My sperm just starts running out without any force. I never really 'GO OFF". I do not hit the climax spot. Any suggestions?

You have Secondary Anorhgasmia

Detailed Answer:
Hello
Thanks for query
Based on the facts that you have posted you seem to have been facing problem of Anorgasmia since very recently

In most of the cases Secondary Anorgasmia is due to following few factors
1) Age related
2)Psychological
3) Low Testosterone levels

Consult qualified Urologist for clinical assessment and get your blood test done to estimate Testosterone levels.

Consult Qualified Psychiatrist for counselling

I can understand age being a factor but I want sex all the time so I cant see having a psychological problem. There is no reason. We are financially secure and we are very happy in our marriage. We've been together over 45 years and have never cheated on each other. I am using testosterone treatment because my body stopped making the hormone when I was in my 40's. There has to be another problem causing this. Both of my grandfathers were sexually active well into their 80's. So I plan on following the same heredity path if I live long enough. I don't understand why this started all at once. Do you have any other ideas? I am getting my testosterone and my PSA checked on Friday at the hospital. I'll let you know the report readings are. Brief Answer:
Please Upload blood reports after you get the same

Detailed Answer:
Hello
Thanks ffor follow up .

Taking Exogenous Testosterone supplements for a long period does reduce natural secretion of Testosterone by Testicle .Please upload reports after you get the same .

The drug Topiramate does have side effects of sexual dysfunction in some patients Please consult your Psychiatrist or Physician who has prescribed you Topiramate to explore the possibility of the same in your case .

Thanks and Regards.

Good Morning. I had my blood tested and the Testosterone was 780 and the PSA was 0.095. So my levels are all in good shape but still does not explain my problem with ejaculation. I am waiting on your thoughts to my problem. Thank You, XXXXXXX Higginbotham

Brief Answer:
Recommended to consult Urologist for a thorough check-up

Detailed Answer:
Hello,
Thanks for the follow-up,

It is difficult to asses your issue.
You need to consult a Urologist for clinical examination in person and to investigate further by doing an MRI of the pelvis.

Do let me know if I can assist you further.

Thanks and Regards
Dr Patil
