Question: Greetings, I have brown discoloration from mid shin down to ankles. It stops where I wear golf socks. It is more prominent in front than back of lower legs. I had a scar from 3rd degree burn from motorcycle pipe (riding in shorts) and that circle scar is not affected. It seems to start where cropped pants (Pants not to ankle, only to mid calf) would stop. It's on both legs. Thought it was a reaction to sun. Hasn't gone away, no red bumps or raised skin. No insurance -= no doctor. Thanks.

The affected skin has a cayenne pepper appearance which is classical of schamburgs purpura. It is due to red blood cells leaking from capillaries and the hemoglobin this released is deposited as hemosiderin giving rise to this appearance. It is a benign condition, restricted to ankle and lower leg region and with no underlying predisposing factors. Various treatment modalities have been tried including topical steroid cream. You may use a moderately potent topical steroid e.g either mometasone furoate 0.1% cream or triamcinolone acetonide 0.1% cream, twice daily.. Some cases may clear spontaneously while others don't clear even with treatment.
