Question: Gyn- Hello Dr. XXXXXXX I have an open sore on my left labia. It is very itchy with three rough white edges. HPV test negative. Negative for Intra Epithelial cells Negative malignancy for lesion. Negative Trich Vaginalis RNA. Pap Normal. All tests normal. My concern is why I have this problem. It looks a little like the picture of rare vaginal cancer. Is vaginal cancer ruled out or should I worry? Thank you.

Brief Answer:
A biopsy is necessary to determine the diagnosis.

Detailed Answer:
Hello dear and welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

I have already reviewed all the examinations that you have already done.

Pap test is the first examination recommended to rule out any abnormal cells.

If we find the presence of abnormal cells, we recommend an HPV test to check for human papilloma virus.

Both of these examinations are normal in your case.

So, the next step that I would recommend is a colposcopy.
It is an examination through a colposcope which examine the vagina.

Based on the information that you have provided the lesion seem to be related to lichen sclerosus. It is a skin disorder which is commonly treated with steroid cream. To determine the diagnosis, a biopsy is necessary. It is an examination through the microscope of a tissue sample to see if it is a cancer or a pre-cancerous condition.

Hope I have answered your query. Let me know if I can assist you further
