Question: HI SIr,
my daughter is 2 year 3 months old.since last 5 days high fever is coming in the span of 4-5 hours.We tried ibugesic plus syrup and antibiotic AZEE 100.but there is no improvement on fever.we tested CRP and Urine.CRP range is very high(202) and also there is infection in urine.SInce last 3 days we have admitted her in hospital.they are giving antibiotic and paracitamal.We have re tested CRP today and its 210 now.
We are awaiting for Blood/Urine Culture report.
Sir please suggest why CRP level is not coming down?Do we need to try any specific antibiotic for it?
Please suggest.
Urine and CRP report is attached.
Now also high fever is coming in every 5-6 hours.

Thanks for asking here

I have gone through the details and do understand your concern.
High grade fever with raised CRP not responding to usual antibiotics can be due to some bacterial infection which is of resistant type or due to some causes other than infection.
Although urine infection is showing 8-10 pulse cells and some protein it will not help much. Urine culture will be of more help.
I would like to know the detail of symptoms like if there is associated cough, cold, rashes, ulcers, eye redness, breathing difficulty, urinary problem, yellow eyes, pain abdomen or any other intestine related issues.
In current situation I would like to get some more investigations done like complete blood count, procalcitonin(for infection), blood and urine culture, ultrasound of abdomen and kidney, ESR(can give clue to some other cause) and chest X-ray (to find any hidden patch in lungs).
As fever is not responding to the current medication I would like to upgrade the antibiotic after sending these investigations.

Hi Sir
I have all the reports now.
Cold and cough is there.eye redness is not there as such.tongue is red as compared to normal.It's difficult to know wheather breathing problem is there or not for my child.Pain in the abdomen is difficult to know for a child also.

Sir please review all the attached reports and suggest the way forward.

Hi

I have seen the reports. The complete blood count shows evidence of infection but CRP has remarkedly decreased and this shows that infection is decreasing.
I hope his fever is better now. The kidney function test is normal but there is mild derangement in liver function. Also the culture report of it urine is negative and USG (both kidney and liver) is not showing any significant abnormality.
Overall to me it appears to be some bacterial infection which has involved liver and kidney.
If fever and general condition is improving I think we should continue the same antibiotics.
After three days repeat cbc, CRP and liver function test should be repeated.
