Had An Injury Back In April To Left Foot. Pain

default

Question: Had an injury Back in April to left foot. Pain in forefoot and midfoot. Dropped 50 lb box on foot. Was given a tall CAM boot and was NWB on crutches for about 6 weeks. During that time started PT and for a steroid injection. Fracture was then found in 2nd metatarsal as displaced. And swelling in 2-4th metatarsals. Had recheck X ray last week with orthopedic surgeon he said there was a fracture seen ( but thinks it was an old one different spot then before) pain has been pretty constant and haven’t been able to wean completely out of CAM boot when walking long distances. At home I usually don’t use as I’m not walking much. Starting yesterday had an increased in pain! And today can’t bare any weight on foot at all. Not sure if I stepped wrong or what but pain is 10/10 when any pressure placed on it! Sharp stabbing pain.
Should I go to EE could
Fracture have gotten worse?

Brief Answer:
You need to attend Emergency room.

Detailed Answer:
Hello.
Thanks for writing to us.
A sharp stabbing pain with inability to bear any weight on the foot with swelling in the area is suggestive of a fracture. The fracture has either got displaced or gotten worse. If there is no fresh injury then an infection also needs to be ruled out. This needs an X- Ray for proper evaluation. You should attend EE and get this properly investigated and get the required prescription medications too.
Hope this answer is helpful.
Any further questions are welcome.

ok great thank you! what would
cause a sharb burning pain when flexing foot where the swelling is at? foot is super tight can barely move it all and when i try i get that burning pain. i have crutches and a cam walking boot should i use that until i can get it evaluated?
the boot hurts it as its hard on top and not soft but it at keast keeps the foot immobile right?
at the ER woukd they cast me or splint me? even if they dont see a fracture? could also me a soft tissue injury as well right? which i would need to NWB as well right?

Brief Answer:
It is better to use a boot till then.

Detailed Answer:
Hello.
Thanks for writing again.
A sharp burning pain can be due to any nerve compression in the area due to the swelling. You can use boot and crutches till you get yourself evaluated.
If there is no fracture then there can be a soft tissue injury. You will have to remain NWB till the pain and swelling subsides.
Hope this answer is helpful.

Brief Answer:
MRI scan will be more helpful.

Detailed Answer:
Hello.
As your pain and swelling are persistent even after NWB it is best to get an MRI scan done to detect any soft tissue abnormality in the area. MRI scan is the best investigation to detect any minor bone related or soft tissue abnormality.
The bruise in the images attached seems to be old blood in the dependent part of the foot and is not worrisome. This will gradually fade away on its own.

Brief Answer:
Lisfranc injury would have been detected on X-ray.

Detailed Answer:
A lisfranc injury also would have been diagnosed on an X-ray. But it is best to get an MRI scan done to be very sure that nothing is being missed.
Bruising on bottom with purplish color suggests an old blood. This color will gradually fade away.
