I have gone through your ultrasound report. There is no gestation sac observed in the uterus, and this means, unfortunately, there is no pregnancy. There are no cysts in the ovary, only multiple follicles are seen. The endometrial thickness does not indicate that ovulation may have occurred. There is no tubal pathology. Please let me know if pregnancy was diagnosed earlier and by what means.
I feel that you are experiencing hormonal imbalance and this could be the reason for your irregular periods. I would suggest you to have a follow up with a specialist and take treatment. If you have had irregular periods in the past, they need to be corrected in order that you may conceive. You may also need ovulation induction. Please maintain a healthy body weight, eat healthy and do some regular daily exercise.

Your sonogram report says that you are not pregnant and the cause of irregular periods should be investigated first. You need not conceive with a single act of unprotected intercourse unless it falls on the day of ovulation. Even if I presume that you have had regular 30-day cycles, the time of intercourse coincides with the safe period and hence, there are no chances of conception. In the current scenario, it is more likely that you are suffering from polycystic ovarian disease and treatment should be directed toward this.

Your doctor has given you medication to induce withdrawal bleeding. If you have conceived, and your pregnancy is viable, your sonogram should definitely show it now. A negative report means that you have no pregnancy now. Withdrawal bleeding is seen 3-7 days after completion of the treatment.
You may need to get investigated for the absence of bleeding. Your symptoms are not specific for pregnancy. Nausea may be seen when there is high estrogen level. You will definitely conceive when the exact cause of disturbance is known. So, please do not worry.

Please consider the values in the non-pregnant state before comparing the levels in the different trimesters of the pregnant state as these criteria vary from individual to individual and from lab to lab. These values are not evidence of pregnancy, but only indicate the function of the thyroid in different states of the body. Your definite indication is the presence of a gestation sac with a fetal pole and fetal heart in the sonogram and nothing else. Please wait and see if you get the withdrawal bleeding, and if you do not get it as expected, see your doctor for further treatment.

I have gone through your sonogram report dated 15th April 2013. The sonologist has given the report as a twin pregnancy but did not mention what is essential, the presence of gestation sacs, their sizes and the evidence of fetal hearts. At 7 weeks, the fetal pole should be at least 5mm. with a fetal heart seen. It is the size of the sac that matters, not the uterus. Please discuss this further with your attending gynecologist and clarify the matter once for all. I can only give you approximations but not definite opinions.
