Brief Answer:
Further Investigations need to be done.

Detailed Answer:
Hello and Welcome to 'Ask A Doctor' service.
I have reviewed your query and here is my advice.

There are three possibilities in your case-

1) Chronic gastroenteritis which causes inflammation in intestines hence with every peristaltic moment there is a spasm in the intestine. Now the bacteria's involved may slowly extend their colonies hence pain involves other quadrants also.

2) Pain related to the female genital system- An ultrasound would be the best test for initial scanning of the ovarian fallopian tube system. To name some Examples of conditions of the reproductive organs to include endometriosis, fibroids, pelvic inflammatory disease, ovarian cysts.

3) Diverticulitis- Diverticulitis is inflammation or infection of small pouches called diverticula that develop along the walls of the intestines.

*Investigations suggested-
Ultrasound abdomen
Stool routine microscopy and culture sensitivity
A CT scan of the abdomen to rule out diverticulitis.

Rx
Therapy with broad-spectrum antibiotics and antispasmodics would be started as per the results.

Hope I was helpful,
Feel free to follow up.

Thanks!
