The primary infection of syphilis can be a single lesion at the site of inoculation or infection. It is called a syphilitic chancre. Most of the time it is painless. Based on what you are describing and the picture you have sent, it is POSSIBLE that this represents syphilis. You need to get this checked out by a doctor. If there is any drainage from the lesion, this can be looked at under a microscope.
