Condition you are describing is called macrohematuria andit means that there is some cause of bleeding in between kidney and urethral opening. Other most common causes are urinary infections(kidney,ureter, bladder or urethra, symptoms are ain while urinating, chills,fever),ureteral stones(very often cause, especially if he experience pain in lumbar area) or any kind of penile or lumbar trauma. Pain in shoulder and legs can be refered pain from urogenital disorders but also many other factors can cause that(heart disease,osteoporosis,diabetes...) ,it is hard to say the exact diagnosis without more info. Also diabetes can affect nerves and make it less sensible to pain so he doesnt need to complain about pain while urinating. Diabetic glomerulonephritis can cause hematuria and diabetes makes people more sensitive to all kind of infections.
I suggest you to do cystoscopy and urography. If anything is found some endoscopic procedures can be done. You should visit urologist and nephrologist both. They can examine genitals and find is there maybe some cause in penis or preputium and do tests i ve mentioned. You must know that hematuria isnt problem for itself. It is just a sign. Man cant bleed out from it. The most important thing is to save kidneys adequate function and check urea and creatinin level frequently.
