Hello dear welcome to ‘Ask A Doctor’ service. I  reviewed your query and here is my advice.

Your family member seems to have a very complex health situation.
These diagnosis are different from each other.
Type 2 diabetes can lead to higher risk of infections which can be related to chronic sinusitis.
Diabetes can cause obesity.
High blood glucose levels, over a number of years, can damage the nerves, and cause difficulty in concentration and decision ability.
In this diagnosis is very important to measure the glycemic level regularly and take proper treatment.

The symptom of behavior switches can be related to substance abuse. I suggest you to consult a psychiatrist for a psychological screening and proper treatment .

Meanwhile, I would suggest :
-a healthy diet including fruits and vegetables
-daily exercise

Hope I have answered your query. Let me know if I can assist you further.
