Hi,
Welcome to XXXXXXX
Dear friend your question required more elaboration, like any history of hypertension, prolonged cough etc.
When ever you lie down your heart is overloaded along with the lungs with inflow of venous blood when ever the heart is not pumping properly . This all may create this situation of breathing difficulty. You may be feeling comfortable when you sitting on the bed. This situation is also called as orthopnea.
In my opinion you should be thoroughly investigated by your physician regarding status of heart /lung by ECG, echocardiography, CXR,and blood sugar level.
Hope your query is answered with your satisfaction.
Please feel free to discuss more.
Thanks

Hi dear
As I suspected her investigation reports are same.
Left Bundle Branch Block, Left Ventricular Hypertrophy with a Ejection Fraction of only 20% are pointing towards compromised and dilated Heart Muscle (some level of Congestive Heart Failure).
All the symptoms related to her can be well explained by such weakened heart muscle.
In my opinion ;
1.Diuretics to be continued with electrolyte maintenance,
2.Heart overload reducing drugs to be continued ,
3.Exertion by her should be avoided.
4.Low salt diet,
5.Avoid Constipation,
6.some Cardiotonic drugs may be considered.
7.Blood pressure to be controlled effectively.
8.Regular watch on Electrolyte Balance.
9.Renal Function to be monitored well.
10.Head End of the patient should be kept high.(propped up position while sleeping)
11.Oxygen support may be considered off and on by simple oxygen mask or some time pressurized oxygen is delivered to patient by a small portable machine. (CPAP).
12.Her Nutrition should be balanced.
12.Supplement of iron and multivitamins will be helpful to her.
Hope your query is answered with your satisfaction and useful to you.
Always ready to clear your doubts.
Thanks
