Brief Answer:
Need expert help 0and self care.for neurogenic pain

Detailed Answer:
Hi Dawnparrez1223,
Thanks for the query.I understand your concern.
"The central bulge of intervertebral disc between L5-S1 with listheasis (tear )followed by inflammation &pressure on nerve roots supplying both legs is the cause of your problem.
* The management in such case can be--
-with pain medication &physiotherapy to correct the harm done with expert advise
- Microscopic surgery if needful to relieve pressure caused by the affected disc.
You need to consult a orthopedic surgeon for specific advise.
* Personal care consists of local fomentation, /avoid forward bending/calcium,vitamin D and vitaminB12suplimentation.
It would be easy to manage the problem with taking personal care as well as following experts advise.
Thanks.

Brief Answer:
Yes, all the symptoms are from vertebral & disc damage.

Detailed Answer:
Hi XXXXXXX Perez,
Thank you for follow up query.
Yes all of your symptoms are due to the deformative changes in your vertebral column.
-The 5th lumbar vertebra has slipped over Sacral 1st vertebra(spondylolisthesis)to cause derangement in spinal curve & mechanism to cause constant back ache.
-This derangement has resulted in central herniation of disc (that acts as a cushion between adjacent vertebrae)to cause compression on both the nerve roots from S1& also narrowing of the foramen (from which the nerve roots come out of spinal cord &the gives nerve supply to affected parts) to cause impingement of L5 nerve roots.
* The compressed nerve roots from L5 &S1.. supply both the lower limbs & these damaged nerve fibers give varied symptoms like pain, burning feeling, tingling ,pins & needles or numbness in the legs --as you have them
** Now the relief would be only after relieving the impinged or compressed nerve roots at spinal level .This can be achieved by oral or injectable drugs, local steroid injection, physiotherapy or surgery as described earlier.
I hope that satisfies you.Any other query you have please don't hesitate to ask.
Thanks.

Brief Answer:
That's be cause few nerve fibers in nerve root are damaged & few are intac

Detailed Answer:
Hi DWnperez,
Thank you for the follow up query.
Images of low back clearly show derrangement in lower vertebrae(L5-S1).&that iOS the cause for your pain.
Nerve roots emerging from spinal foramina contain multiple nerve fibers for different sences&motor function.
EMG &NCV shows electromagnetic readings in muscles ,because motor nerve fibers in the nerve root(near vertibrae)are not damaged by the impingement .&your motor activity is intact. But few sensory fibers have been damaged .So you have disturbed sences causing numbness, tingling & pain .& the nerve is carrying out motor function( carrying out movements) &so both EMG&NCV tests are giving positive result inspite of the damage .
Your problem is due to nerve compression only...so my advise in previous answer is maintened.
I hope that helps.Any more doubt, I am eager to answer.
Thanks.

Detailed answer below.

Detailed Answer:
Hi,

Thanks for asking.

- UTI needs to be excluded by routine µscopic/culture &sensitivity of irine along with detailed physical examination
- Cloudy urine with frequent urination can be there when the urine is concentrated(dehydration)/ Whenever there pus or blood discharge in urine./when there is vulvobaginal infection to cause vaginal discharge to mixe with urine.
- Some times diabetes can present with the complaints like you have.
- Vulvovaginitis itka che to bacterial or fungal or sexually transmitted diseases can lead to same symptoms.
- Urinary stones can cause the same symptoms.
- Irritation/inflammation of urethral meatus .vagina or urinary bladder can cause the symptoms.
*All of these conditions cause frequent urination too.
*For self care you should start drinking plenty of water.
Follow good genital hygiene.
* consultation of your family doctor for primary evaluation &investigations would throw some light on the cause.Followed by specific management OR referral to specialist.
I hope that answers your query.Any follow up query is welcome.
Thanks.

Brief Answer:
Mostly nurogenic

Detailed Answer:
Hi XXXXXXX Pereze1223,
Thank you for the follow up query. &additional information that makes many things clear.
I feel even in these circumstances infection(which has not been ruled out ) OR nurogenic symptom due to your L5,S1 disc problem...seems to be the cause.
I feel simple tests of urine routine /culture if needful & vaginal /urethral examination should rule out infection-irritation or urinary stones.
Then treating your problem would be Solely management of the spinal injury.under care of a spinal surgeon. The residual urine &discomfort of abdomen as a sequel ,also discomfort at the urethra would be attributed to spinal injury.
I hope that helps you.Anyfollow up query is welcome.
Thanks
