Having Fever And Weakness. Blood Test Showed Elevated Alkaline Phosphatase. Any Thoughts?

Question: Hi, my wife aged 27 has been having fever and weekness for the last 3 days. After consultation they have asked us to do lft and bilerubin test. The results serum bilerubin 1.8mg% , sgpt :235, sgot 306 u/l. Serum alkaline phosphate :321 u/l. Total protein 6.2 gms%, serum albumin 4.6, serum globulin 1.6 gms. Is this jaundice. If it is what test are required to confirm what hepatitis it is. A,b,c,e....isthis level alarming.

Your wife results indicate that she has jaundice indeed. However, as alkaline phosphatase is elevated as well as bilirubin, most likely the reason of such results is bile ducts obstruction due to gallstones. The other symptoms of this conditions are upper right abdominal pain radiating to the back, nausea, vomiting, dark urine, whitish stool. If this fits to your wife, she should have abdominal ultrasound performed to see if bile ducts are widen, and if so, ERCP procedure should be done.
If no findings in ultrasound, she may have be tested for hepatitis viruses. To do that HbSAg and Anti-HCV antibodies should be checked.
