Question: Having gender reassignment surgery tomorrow morning at 1030. We're in CST. The regular bowel cleansing methods are not working. At the pre-op checkup they said not to use suppositories or enema (saline) the night before. She's in so much pain. She's been following the regimen for the last four hours and has had no movement and is in so much pain.

Hello and welcome to Ask A Doctor services.
I have read your query and here is my answer.

* As per my clinical experience, oral molecules available in forms of
polyethylene glycol, sodium picosulfate and other supportive purgatives
are being tried till now with no bowel movement.

* In this condition, there can be two possibilities as

- Bowel is not loaded with stool content.
- Medicine is not that responsive as it should be or can predict delayed effect
within next 4-5 hours.

* Ideal condition is to empty colon by purgative suppository or enema , but
during pre op check up it has been said that they should not be used.

Now in this condition, as per my surgical experience, I would suggest that

- Pain relief can be best obtained with tablet dicyclomine and paracetamol
with some sort of snacks which will relieve intestine colic within 30
minutes of intake.
- For bowel evacuation should consult doc office representative to get further
guidelines ( better option ) against waiting for further 4-5 hours to get
effects of oral molecules already consumed.
