Thank you for your query & hope to answer it best I can, given your brief yet concise history.

At the outset, note you have a Blood pressure issue. This problem is unrelated to this issue.

Assuming this spot has been on for days & on the upper lip & itchy, it is more than likely to be an 'irritant' or 'contact dermatitis' reaction to some allergen most likely some local application eg. Hair coloring or dyes used in region or citrus fruits (lemon, sweets sucked etc) that are commonly incriminated agents.

Alternatively as spreading, may be a 'fungal infection'. Do use a topical steroid, preferably one that has an antifungal element in it for 5-7 days twice daily & this should resolve the problem.
