Question: Hello. 55 yo female. I've had pinch like pain upper right quadrant under right rib on and off after drinking too much alcohol (3 drinks/day but some days more) for several years. Pain was lasting seconds and go away. I thought liver inflammation or adhesions, pain was like where gallbladder used to be. I've had reflux for years, treated with rolaids. Pain under right rib got worst around December 23rd, very gassy, lots of mushy stools, felt like something under rib and pain was also moving to the back on the right. Started taking Pantoprazole 40mg 1X/day Dec 23rd. Pain and bloating was gone last week, stopped taking medication and small twinge returned, I resumed medication. Pain is pretty much gone now and stools consistency is also a lot better. I was worried about liver. (Had full medical exam and blood work sep 2018 and all looked normal ) 09 January, felt dizzy when got up morning but got better right away. Later, I had terrible headache!! 10 XXXXXXX small headache remaining, not enough sleep, woke up 12 XXXXXXX scratchy throat, VERY tired, sore bones. Last night, not enough sleep, I have tinnitus but last 2 days it's really bad, trouble falling asleep. Today, cough a bit from chest, tired and scratchy throat. I wonder if the tiredness and sleeplessness are related to stomach problem or just a cold/flu. I read that a side effect of pantoprazole is tiredness but been taking it since Dec 23 and this just started. Also worry that would tiredness be my body trying to heal stomach? It feels more like a cold or flu virus but worry a bit. Seeing doctor tomorrow. The other question, is if stomach feeling much better, does it mean whatever was going on (other doctor online said likely gastritis) has healed?

Brief Answer:
Viral pharyngitis very likely.

Detailed Answer:
Hello and thanks for choosing "Ask a Doctor" service for your query,
Have seen your details and I appreciate your concerns,
Firstly your current problem is neither related to pantoprazole side effects or Gastritis.History of cough with tinnitus is suggestive of pharyngitis/Upper respiratory tract infection.
You have to get yourself investigated by your doctor for that. He would look for soreness in throat would auscultate by stethoscope to hear for the sounds of lung.
Secondly Gastritis can't be cured unless it is caused by a bacteria named H.pylori.Strict dietary control with regular antacids are required to keep Gastritis in control. Its fully controllable but non curable.
Just taking pantoprazole for few days and then cold turkey may cause even rebound gastritis.

Fatigue etc is due to pharyngitis/Upper respiratory tract infection.Your doctor would prescribe you medication for the same. Usually it's caused by virus and treatment is not required( symptomatic treatments is required).

Pantoprazole 40 mg is too low a dose to cause side effects. However in rare of rare cases they may cause minor side effect those patients can switch over to H 2 blockers like Ranitidine 150 mg twice daily.Well I don't think that pantoprazole has any relation to problems in your case.
As per protocol Antacids are tapered off very slowly. Cold turkey is usually not suggested and usually symptoms revert back with full intensity.

Formation of excessive gas means that there is something wrong with your digestive system. Food reaches intestines and is acted upon by bad bacteria's causing incomplete digestion of food(Fermentation) releasing large amount of toxic gases which may mimic symptoms of gastritis.
This toxic gas kills colonies of good bacteria's required for proper food digestion hence you need to add over the counter product like Probiotic and activated charcoal tablets( to absorb gas).

Proper diet is the key otherwise you need to take antacids regularly.
Hope I answered your query well,
Follow ups are welcome,
Thanks.
